2 Lckut/Tagut Prgram 1.0 INTRODUCTION 1.1 Plicy It is the plicy f the University f Califrnia, San Dieg (UCSD) t ensure a safe and healthful wrk envirnment fr faculty, staff, students and visitrs. Implicit in this plicy is a requirement t prvide individuals with pertinent infrmatin abut Lckut/Tagut prcedures. 1.2 Purpse The purpse f the Lckut/Tagut (LOTO) Prgram is t ensure specific practices and prcedures t safeguard emplyees frm the unexpected startup f machinery and equipment r the release f hazardus energy during service r maintenance activities as specified in Cal/OSHA Tile 8 Sectins 3203, 3314 and Requirements fr wrking n energized electrical systems are prescribed in Sectins , , , and f the electrical safety rders. This prgram requires that an authrized emplyee(s) turns ff and discnnects the machinery r equipment frm its energy surce(s) befre perfrming service r maintenance. The authrized emplyee(s) als either lcks ut, blcks ut and/r tags ut the energy-islating device(s) t prevent the release f hazardus energy and take steps t verify that the energy has been islated effectively. 1.3 Scpe This prgram prvides guidelines in the servicing and maintenance f machines and equipment in which the unexpected energizatin r start-up f the machines r equipment, r release f stred energy culd cause injury t emplyees. This prgram establishes minimum perfrmance requirements fr the cntrl f such hazardus energy. LOTO applies t all facility and labratry equipment. LOTO prcedures apply t the fllwing frms f energy: _ Electrical _ Gravity _ Pneumatic _ Springs _ Hydraulic _ Steam _ Heat _ Thermal _ Psitinal r stred _ Chemical _ Natural Gas _ Other LOTO des nt apply t: Wrk n crd and plug cnnected electrical equipment where the equipment is unplugged, and the plug is under the exclusive cntrl f the emplyee wh is perfrming the servicing and maintenance n the equipment. Fr example hand pwer tls and plug/crd cnnected equipment. Hwever, if at any time the emplyee n lnger has direct cntrl f the plug/crd cnnected equipment, then LOTO prcedures wuld apply. During Ht Tap peratins that invlve transmissin and distributin systems fr gas, steam r water when perfrmed n pressurized pipelines. NOTE: This functin is nly t be perfrmed by a qualified/authrized steam fitter defined by industry standards.

3 Lckut/Tagut Prgram When Lckut/Tagut prcedures intrduce additinal hazards int the wrk being perfrmed, justificatin fr perfrming such actins must be established and a site safety plan must be develped and adhered t. 2.0 RESPONSIBILITY Each department that prvides servicing and maintenance f machines and equipment r units is respnsible fr develping written machine specific Lckut/Tagut prcedures. Where written prcedures have nt been fully develped, blue prints r as-built drawings must be available t identify surces f energy and sufficient Lckut/Tagut cntrl pints. 2.1 Supervisr Supervisrs are respnsible fr: Identifying all machinery r equipment that wuld be subject t this prgram. Develpment, dcumentatin and implementatin f written energy cntrl prcedures fr each ptentially hazardus energy surce. Identifying authrized and affected emplyees within their department. Ensuring that authrized and affected emplyees receive apprpriate training. Ensuring that all new r refurbished equipment is capable f accmmdating lckut devices. Evaluating and Crrecting deficiencies identified in the Lckut/Tagut prgram. Prviding nging supprt f the Lckut/Tagut prgram. Managing and tracking equipment that is in a Lckut/Tagut state. Disclsing identified hazardus cnditins t Cntractrs wrking at UCSD. 2.2 Emplyee Emplyees are respnsible fr: Fllwing the written prcedures. Assisting with identificatin f hazardus energy surces. Reprting knwn r suspected deficiencies t their supervisr. Prviding Supervisrs with the peratinal status f equipment under Lckut/Tagut. Participating in training fr Lckut/Tagut. 2.3 Department f Envirnment, Health and Safety (EH&S) EH&S is respnsible fr: Develping and maintaining a written prgram which cmplies with the requirements f CCR Title , The Cntrl f Hazardus Energy fr the cleaning, repairing, servicing, setting-up, and adjusting peratins f prime mvers, machinery and equipment, including Lckut/Tagut. Prviding training t Supervisrs/Department Heads n the requirements f the prgram. Assisting in identificatin f hazardus energy surces and the selectin f prper energy cntrl devices. Perfrming peridic assessments t identify deficiencies in the LOTO prgram.

4 Lckut/Tagut Prgram 3.0 IDENTIFICATION OF ENERGY SOURCES UCSD Maintenance and research departments, which have hazardus energy surces, must identify all machinery and equipment that is subject t LOTO. Panel schedules will be maintained and updated by the department r cntractr perfrming any mdificatins r wrk in the area. 4.0 ENERGY CONTROL PROCEDURES Each written prcedure fr the cntrl f hazardus energy will cntain the fllwing steps. 4.1 Lckut/Tagut f Equipment Prcedures When servicing r maintenance f machinery r equipment is required, the fllwing steps shall be taken. Ntify affected emplyees. The authrized emplyee shall identify the type(s) and magnitude(s) f energy that the machine r equipment uses. The authrized emplyee shall understand the hazards f the energy, and shall knw the methds t cntrl the energy. If the machine r equipment is perating, shut it dwn by the nrmal stpping prcedure (depress the stp buttn, pen switch, clse valve, etc.). De-activate the energy islating devices(s) s that the machine r equipment is islated frm the energy surce(s). Lck ut the energy islating device(s) with assigned individual lck(s) and tags. Stred r residual energy (such as that in capacitrs, springs, elevated machine members, rtating flywheels, hydraulic systems, air, gas, steam, water pressure, etc.) must be discharged, dissipated r restrained by methds such as grunding, repsitining, blcking, blanking r bleeding dwn t remve r islate stred energy ptentials. Ensure that the equipment is discnnected frm the energy surce(s) by first checking that n persnnel are expsed, then verify the islatin f the equipment by perating the start n/ff buttn r ther nrmal perating cntrl(s) r by testing t make certain the equipment will nt energize r perate. Cautin: Return perating cntrl(s) t neutral r "ff" psitin after verifying the islatin f the equipment. The machine r equipment is nw lcked ut. Remve machinery guards r access panels necessary t perfrm wrk. 4.2 Restring Equipment T Service When servicing r maintenance is cmpleted and the machine r equipment is ready t return t nrmal perating cnditin, the fllwing steps shall be taken. Check the machine r equipment and the immediate area arund the machine t ensure that nnessential items have been remved and that the machine r equipment cmpnents are peratinally intact, machinery guarding in place, etc. Check the wrk area t ensure that all emplyees have been safely psitined r remved frm the area. Verify that the cntrls are in neutral.

5 Lckut/Tagut Prgram Remve the lckut devices and re-energize the machine r equipment. NOTE: The remval f sme frms f blcking may require the machine t be re-energized befre safe remval. Precautins must be taken t ensure emplyees are nt expsed t rtating r mving parts when this prcess is perfrmed. Ntify affected emplyees that the servicing r maintenance is cmpleted and the machine r equipment is ready fr use. Inspect wrk area and assure that all tls and peple are clear frm equipment r machinery. Replace all guards. Ntify affected emplyees in the immediate area. Remve all energy islating devices. If mre than ne individual is required t lckut the equipment, each persn will place a persnal lck and tag n a grup lckut device prir t beginning wrk. Each individual applying a lck is respnsible fr its remval upn cmpletin f their task. During shift changes, the supervisr shall ensure the cntinuity f the lckut prtectin. Wrkers will be respnsible fr remving their wn lck and tag at the cmpletin f the shift. If wrk is t cease until the fllwing day, the emplyee may leave his lck n the equipment r the supervisr may replace it with his persnal lck and tag s that the emplyees may remve their lck. When wrk resumes, the emplyees are t place their lckut n the equipment prir t the remval f the supervisr s lckut. 5.0 LOCKOUT/TAGOUT DEVICES Lckut devices are the preferred methd fr energy islatin. In the event that machinery r equipment is nt capable f being lcked ut a Tagut prcess may be used. Due t the fact that tags may be easily remved, bypassed r ignred, further steps must be taken t ensure cntrl f hazardus energy. All Lckut/Tagut devices must meet the fllwing requirements: Emplyees must be prvided with sufficient numbers f devices fr cntrl f hazardus energy. Emplyee will apply assigned lcks/tags n all energy cntrl devices capable f accepting lckut devices at all times. Devices must indicate the identity f the emplyee applying the device, the date it was applied, cntact number and shall warn against hazardus cnditins if the machine is energized. These devices must nly be used fr cntrlling energy. There will be nly ne key in use fr each lckut lck. Lckut/Tagut devices must be capable f withstanding the envirnment fr the perid f time they will be applied. Devices must be standardized within the University by clr, shape r size. Lckut devices must be sturdy enugh t prevent remval withut excessive use f frce. Tagut devices must be self-lcking and capable f withstanding at least 50lbs f frce. 6.0 ASSESSMENT T ensure that the prgram is being implemented and prviding adequate prtectin fr emplyees, an assessment/evaluatin will be cnducted annually. The assessment will be cnducted by the supervisr, department head r manager, and when requested a member f the EH&S staff wh is knwledgeable abut LOTO prcedures.

6 Lckut/Tagut Prgram The assessment will be designed t identify and crrect deficiencies. During the assessment, prcedures will be reviewed by authrized emplyees wh will assist in the identificatin f deficiencies and the develpment f slutins. Dcumentatin f the assessment will include the machine r equipment evaluated the date f the assessment, the peple invlved and any crrective actin taken. These recrds will be kept fr at least five (5) years in the department ffice. 7.0 TRAINING The department, supprted by EH&S, will be respnsible fr training t ensure the purpse and functin f the LOTO prgram is understd by all authrized and affected emplyees. Authrized emplyees will receive training in the recgnitin f applicable hazardus energy surces, the type and magnitude f the energy available in the wrkplace, and the methds and means necessary fr energy islatin and cntrl. The training will be dcumented n Frm #2 in Appendix III. Affected emplyees will be instructed in the purpse and use f the energy cntrl prcedure. The training/instructin will be dcumented n Frm #3 in Appendix III. Other emplyees whse wrk peratins are r may be in an area where energy cntrl prcedures may be utilized, will be instructed abut the prcedure and abut the prhibitin relating t attempts t restart r re-energize machines r equipment which are lcked ut. The instructin will be dcumented n Frm #4 in Appendix III. Retraining will be cnducted when emplyees n lnger demnstrate an understanding r prficiency with the LOTO prgram, when jb assignments change, when equipment r prcesses are mdified r when new equipment is installed. In the event that Tagut devices must be utilized the training will include the fllwing: Tags are warning devices they d nt prvide any type f physical restraint. Tags cannt be remved ther than by the authrized persn wh applied the tag and must fllw the same steps when remving a lckut device. Tags must be legible and understandable. Tags must be made ut f material that is capable f withstanding the envirnment in which they are used. Tags must be securely attached. 8.0 CONTRACTORS Cntractrs must have a written LOTO Prgram in place that is as stringent as the UCSD prgram. They will fllw their LOTO Prgram and crdinate all LOTO activities with Facilities Management r Department Management persnnel. Upn cmpletin f installatins and mdificatins f machinery r equipment, the cntractr will submit blue prints, schematics, LOTO prcedures r ther data as specified by the hiring department. This shall be dne within the timeframe established by the hiring department r upn prject cmpletin.

7 Lckut/Tagut Prgram 9.0 MANAGEMENT REMOVAL OF LOCKOUT/TAGOUT DEVICES In the event a Lckut/Tagut device must be remved t cntinue wrk, the persn applying the device must be the ne t d s. Shuld a situatin arise that the persn applying the device is nt available the lck may nly be remved after the fllwing cnditins are met. Every attempt has been made t lcate the persn. Verificatin that the emplyee is nt n the premises and is nt able t return. Shuld the individual be lcated he/she must return t the site t remve the device. The supervisr and a member f upper management agree that the device must be remved. The device is remved in accrdance with the Lckut/Tagut prgram sectin 4.2 Restring Equipment T Service. Dcumentatin (Appendix I) f this prcess will be kept n file in the department fr five (5) years and evaluated during the annual assessment GLOSSARY OF TERMS Affected emplyee - An emplyee whse jb requires him/her t perate r use a machine r equipment n which servicing r maintenance is being perfrmed under Lckut r Tagut, r whse jb requires him/her t wrk in an area in which said servicing r maintenance is being perfrmed. Authrized emplyee - A persn wh lcks ut r tags ut machines r equipment in rder t perfrm servicing r maintenance n them. An affected emplyee becmes an authrized emplyee when that emplyee's duties include perfrming servicing r maintenance cvered under this sectin. Blind - A device such as metal plates inserted in flanged jints, blind flanges, and caps r plugs installed n the ends f pipe lines, used t prevent liquids, gases, r vaprs frm passing thrugh a pipe line r nzzle. Valves are nt blinds. Blck Out - A device r prcess which prvides physical supprt r a barrier t prevent incidental rtatin, shifting, mving r displacement f an islated piece f equipment due t gravity, wind, seismic activity r ther unplanned events that culd cause mvement f the equipment. Capable f being lcked ut - An energy islating device is capable f being lcked ut if it has a hasp r ther means f attachment t which, r thrugh which, a lck can be affixed, r it has a lcking mechanism built int it. Other energy islating devices are capable f being lcked ut, if lckut can be achieved withut the need t dismantle, rebuild, r replace the energy islating device r permanently alter its energy cntrl capability. Energized - Cnnected t an energy surce r cntaining residual r stred energy. Types f energy include but are nt limited t: _ Pressurized fluid (air, il, water, etc.) _ Electrical _ Freely mving machine members _ Ptential/stred Energy (spring, gravity, etc.) Energy islating device - A mechanical device that physically prevents the transmissin r release f energy, including but nt limited t the fllwing: A manually perated electrical circuit breaker, a discnnect switch, a manually perated

8 Lckut/Tagut Prgram switch by which the cnductrs f a circuit can be discnnected frm all ungrunded supply cnductrs, and in additin, n ple can be perated independently; a line valve, a blck, and any similar device used t blck r islate energy. Push buttns, selectr switches and ther cntrl circuit type devices are nt energy islating devices. Energy surce - Any surce f electrical, mechanical, hydraulic, pneumatic, chemical, thermal, r ther energy. Ht tap - A prcedure used in the repair, maintenance and services activities which invlves welding n a piece f equipment (pipelines, vessels r tanks) under pressure, in rder t install cnnectins r appurtenances. It is cmmnly used t replace r add sectins f pipeline withut the interruptin f service fr air, gas, water, steam, and petrchemical distributin systems. Lckut - The placement f a lckut device n an energy islating device, in accrdance with an established prcedure which ensures that the energy islating device and the equipment being cntrlled cannt be perated until the lckut device is remved. Lckut device - A device that utilizes a psitive means such as a lck, either key r cmbinatin type, t hld an energy islating device in the safe psitin and prevent the energizing f a machine r equipment. Included are blank flanges and blted slip blinds. Nrmal prductin peratins - The utilizatin f a machine r equipment t perfrm its intended prductin functin. Servicing and/r maintenance - Wrkplace activities such as cnstructing, installing, setting up, adjusting, inspecting, mdifying, maintaining and/r servicing machines r equipment. These activities include lubricatin, cleaning r unjamming f machines r equipment and making adjustments r tl changes, where the emplyee may be expsed t the unexpected energizatin r startup f the equipment r release f hazardus energy. Setting up - Any wrk perfrmed t prepare a machine r equipment t perfrm its nrmal prductin peratin. Tagut - The placement f a Tagut device n an energy islating device, in accrdance with an established prcedure, t indicate that the energy islating device and the equipment being cntrlled may nt be perated until the Tagut device is remved. Tagut device - A prminent warning device, such as a tag and a means f attachment, which can be securely fastened t an energy islating device in accrdance with an established prcedure, t indicate that the energy islating device and the equipment being cntrlled may nt be perated until the Tagut device is remved.
