2 Versin: 5 Page: 2 f Intrductin 1.1 Purpse The purpse f this prgram is t prvide guidance in the prtectin f all Emry emplyees frm the unexpected energizing, start up, r release f stred energy that may ccur during the servicing r maintenance f machines and equipment as prescribed in the Occupatinal Safety and Health Administratin (OSHA) standard 29 CFR The Cntrl f Hazardus Energy (lckut/tagut). While sme entities and/r divisins f Emry may have additinal r mre stringent guidelines, the guidelines utlined in this dcument shall serve as the minimum requirements fr all. 1.2 Scpe This prgram is inclusive f all Emry emplyees, including healthcare, faculty, staff, student emplyees, cntractrs, and ther peple wh perate, use, r wrk in clse prximity t machines r equipment n which servicing r maintenance is being perfrmed under lckut r tagut. 1.3 Definitins Affected Emplyee. An emplyee whse jb requires him/her t perate r use a machine r equipment n which servicing r maintenance is being perfrmed under lckut r tagut, r whse jb requires him/her t wrk in an area in which such servicing r maintenance is being perfrmed. Authrized Emplyee. A persn wh lcks ut r tags ut machines r equipment in rder t perfrm servicing r maintenance n that machine r equipment. An affected emplyee becmes an authrized emplyee when that emplyee's duties include perfrming servicing r maintenance f equipment that is required t be lcked ut r tagged ut. Capable f being Lcked Out. An energy islating device is capable f being lcked ut if it has a hasp r ther means f attachment t which, r thrugh which, a lck can be affixed, r it has a lcking mechanism built int it. Other energy islating devices are capable f being lcked ut if lckut can be achieved withut the need t dismantle, rebuild, r replace the energy islating device r permanently alter its energy cntrl capability. Energized. Cnnected t an energy surce r cntaining residual r stred energy. Energy Islating Device. A mechanical device that physically prevents the transmissin r release f energy, including but nt limited t the fllwing: A manually perated electrical circuit breaker; a discnnect switch; a manually perated switch by which the cnductrs f a circuit can be discnnected frm all ungrunded supply cnductrs, and, in additin, n ple can be perated independently; a line valve; a blck; and any similar device used t blck r islate energy. Push buttns, selectr switches, and ther cntrl circuit type devices are nt energy islating devices. Energy Surce. Any surce f electrical, mechanical, hydraulic, pneumatic, chemical, thermal, r ther energy.

The placement f a lckut device n an energy islating device, in accrdance with an established prcedure, ensuring that the energy islating device and the equipment being cntrlled cannt be perated until the lckut device is remved. Lckut Device. A device that utilizes a psitive means such as a lck, either key r cmbinatin type, t hld an energy islating device in the safe psitin and prevent the energizing f a machine r equipment. Included are blank flanges and blted slip blinds. Servicing and Maintenance. Wrkplace activities such as cnstructing, installing, setting up, adjusting, inspecting, mdifying, and maintaining and/r servicing machines r equipment. These activities include lubricatin, cleaning r unjamming f machines r equipment and making adjustments r tl changes, where the emplyee may be expsed t the unexpected. Tagut. The placement f a tagut device n an energy islating device, in accrdance with an established prcedure, t indicate that the energy islating device and the equipment being cntrlled may nt be perated until the tagut device is remved. Tagut Device. A prminent warning device, such as a tag and a means f attachment, which can be securely fastened t an energy islating device in accrdance with an established prcedure, t indicate that the energy islating device and the equipment being cntrlled may nt be perated until the tagut device is remved. 1.4 Respnsibilities Envirnmental Health and Safety Office (EHSO) and Applicable Hspital and Clinic Safety Management As the administrative department fr the Energy Cntrl Prgram, EHSO and applicable hspital and clinic safety management are respnsible fr: Develpment, implementatin, and administratin f the Energy Cntrl Prgram; Assisting supervisrs with develpment f specific lckut/tagut prcedures; Reviewing, updating, and evaluating the verall effectiveness f the Energy Cntrl Prgram; Ensuring that Lckut/Tagut training is prvided t all Emry emplyees. Directrs, Supervisrs and Managers Directrs, supervisrs, and managers have primary respnsibility fr the management and enfrcement f the Energy Cntrl Prgram in their areas. They must: Prduce an inventry f equipment and machines that have the ptential fr unexpected energizatin, start-up, r release f stred energy; Develp lckut/tagut prcedures, with the assistance f the EHSO and/r applicable hspital and clinic safety management, fr all equipment in which the unexpected energizatin r start-up f the equipment, r release f stred energy, culd harm emplyees; Ensure that all affected persnnel are trained; Submit an Energy Cntrl Prcedure Frm t EHSO r the applicable hspital and clinic safety management whenever a new piece f equipment r machinery is intrduced int the wrk area; Ensure an annual review f lckut / tagut prcedures is cnducted with

Emplyees All emplyees are respnsible fr cmplying with the rules set frth by this prgram and must cmplete all required Energy Cntrl Training; If wrk must be perfrmed n equipment that des nt have a specific written prcedure, emplyees must lck ut the equipment and infrm the supervisr. 1.5 Training Requirements EHSO and the applicable hspital and clinic safety management are respnsible fr ensuring that Energy Cntrl training is prvided t their emplyees wh wrk with (r in clse prximity t) machines r equipment that will be lcked ut r tagged ut. This training is given t authrized and affected emplyees upn initial assignment and whenever there is a change in the emplyee s jb assignments, a change in machines, equipment r prcesses that present a new hazard, r when there is a change in the energy cntrl prcedures. It is the respnsibility f the supervisr t ensure their emplyees cmplete training, infrm EHSO and the applicable hspital and clinic safety management when a new emplyee is assigned, and when new equipment is intrduced int the wrk area. Training will be cnducted by EHSO and the applicable hspital and clinic safety management. EHSO and the applicable hspital and clinic safety management will maintain dcumentatin f attendance (including emplyee s name, department, and signature) fr trainings they prvide their emplyees. The training will include the fllwing: The recgnitin f applicable hazardus energy surces; The type and magnitude f the energy available in the wrkplace; The methds and means necessary fr energy islatin and cntrl; and Instructin in the purpse and use f the energy cntrl prcedure. When tagut systems are used, emplyees als are trained in the fllwing: Tags must be securely attached t energy islating devices s that they cannt be inadvertently r accidentally detached during use. Tags are nly warning devices; Tags d nt prvide the physical restraint that is prvided by a lck; Tags cannt be remved withut authrizatin f the authrized persn respnsible fr it; Tags are never t be bypassed r ignred; Tags must be legible and understandable by all authrized emplyees, affected emplyees, and all ther emplyees whse wrk peratins in the area; Tags and their means f attachment must be made f materials which will withstand the envirnmental cnditins encuntered in the wrkplace. Additinal retraining is cnducted whenever a peridic inspectin reveals r whenever management has reasn t believe, that there are deviatins frm r

5 Versin: 5 Page: 5 f 8 inadequacies in the emplyee s knwledge r use f the energy cntrl prcedures. The retraining reestablishes emplyee prficiency and intrduces new r revised cntrl methds and prcedures, as necessary. 1.6 Recrdkeeping Requirements All training recrds are retained fr tw years and cntain emplyee s name and date f training. Training recrds fr trainings cnducted by EHSO are retained by EHSO Training recrds fr training cnducted by hspital and clinic safety management are retained by the applicable hspital and clinic safety management. 1.7 Prgram Evaluatin The written Energy Cntrl Prgram shall be re-evaluated peridically, at a minimum f every tw years, and will be revised as necessary. 2.0 Prerequisites fr Lckut/Tagut De-energize the equipment r machine and lckut r tagut prir t servicing r perfrming maintenance n energized equipment r machine; NOTE: Only the authrized emplyee perfrming the servicing r maintenance f the equipment may perfrm lckut r tagut; Utilize lckut prcedures if an energy islating device is capable f being lcked ut. Hwever, if it can be demnstrated that the utilizatin f a tagut system will prvide full emplyee prtectin, yu may use the tagut system. Use a tagut system if an energy islating device is nt capable f being lcked ut; When a tagut device is used n an energy islating device, which is capable f being lcked ut, the tagut device is attached at the same lcatin that the lckut device wuld have been attached. 3.0 Energy Cntrl Prcedures Prcedures must be develped, dcumented, and utilized fr the cntrl f ptentially hazardus energy when emplyees are engaged in the activities cvered by this Prgram. The prcedures clearly and specifically utline the scpe, purpse, authrizatin, rules, and techniques t be utilized fr the cntrl f hazardus energy, including - but nt limited t - the fllwing: A specific statement f the intended use f the prcedure; Specific prcedural steps fr shutting dwn, islating, blcking, and securing machines r equipment t cntrl hazardus energy; Specific prcedural steps fr the placement, remval, and transfer f lckut devices r tagut devices and the respnsibility fr them; and Specific requirements fr testing a machine r equipment t determine and verify the effectiveness f lckut devices, tagut devices, and ther energy cntrl measures. The prcedures require the supervisr t submit an Energy Cntrl Prcedure Frm t EHSO r applicable hspital and clinic safety management whenever a

6 Versin: 5 Page: 6 f 8 new piece f equipment r machinery is intrduced int the wrk area. NOTE: If an emplyee must perfrm wrk n a piece f equipment that des nt have a specific prcedure, the emplyee must lck ut the equipment and infrm the Supervisr. The Supervisr is respnsible fr submitting the Energy Cntrl Prcedure frm t EHSO r applicable hspital and clinic management persnnel. 4.0 Prtective Materials and Hardware The emplyer prvides lcks and tags fr the islating, securing, r blcking f machines r equipment frm energy surces. Bth lckut devices and tagut devices indicate the identity f the emplyee applying the device(s). Lckut and tagut devices are standardized within the facility. Lcks used fr lckut/tagut are nt used fr any ther purpse. Lcks must be capable f withstanding the envirnment t which they are expsed fr the maximum perid f time that expsure is expected. Tagut devices warn against hazardus cnditins if the machine r equipment is energized and include a legend such as the fllwing: D Nt Start. D Nt Open. D Nt Clse. D Nt Energize. D Nt Operate. Tagut devices are cnstructed and printed s that expsure t weather cnditins r wet and damp lcatins will nt cause the tag t deterirate r the message n the tag t becme illegible. Tagut devices, including their means f attachment, are substantial enugh t prevent inadvertent r accidental remval. Tagut device attachment means are f a nn-reusable type, attachable by hand, self-lcking, and nn-releasable with a minimum unlcking strength f n less than 50 punds and having the general design and basic characteristics f being at least equivalent t a nepiece, all envirnment-tlerant nyln cable tie. 5.0 Peridic Inspectin Supervisrs are respnsible fr cnducting annual inspectins f the Energy Cntrl Prcedures t ensure that the prcedure and the requirements f this prgram are being fllwed. The annual inspectins cnsist f the fllwing: A review f the Energy Cntrl Prcedures t determine if any deviatins r inadequacies exist. A review f the emplyee s respnsibilities under the energy cntrl prcedure being inspected and the elements set frth in the Energy Cntrl Standard (The cntrl f hazardus energy (lckut/tagut) ). The Energy Cntrl Prcedure is revised if the review f the prcedure indicates that the current prcedure is ineffective. Fr each review, the emplyees sign a Lckut/Tagut Review Sign-in Sheet which includes the fllwing: Identificatin f the machine r equipment n which the energy cntrl prcedure is utilized, and The date f the review, the emplyee(s) included in the review, and the

The sign-in sheet is retained by the department cnducting the review and is available fr inspectin by EHSO r applicable hspital and clinic safety management. 6.0 Ntificatin f Emplyees Affected emplyees are ntified by the supervisr r authrized emplyee f the applicatin and remval f lckut devices r tagut devices. Ntificatin is given befre the cntrls are applied and after they are remved frm the machine r equipment. 7.0 Release frm Lckut/Tagut Befre lckut r tagut devices are remved and energy is restred t the machine r equipment, prcedures are fllwed and the fllwing actins are taken by the authrized emplyee(s): The wrk area is inspected t ensure that nnessential items have been remved and t ensure that machine r equipment cmpnents are peratinally intact. The wrk area is checked t ensure that all emplyees are safely psitined r remved. Affected emplyees are ntified that the lckut r tagut device(s) have been remved prir t the equipment being restarted. Nte: The emplyee wh applied the lckut r tagut device t the energy islating device is the ne wh remves the device. When the authrized emplyee wh applied the lckut r tagut device is nt available t remve it, that device may be remved under the directin f the area supervisr and in accrdance with the fllwing prcedure: The area supervisr r his designee must verify that the authrized emplyee is nt n campus. The area supervisr r his designee will make every attempt t cntact the authrized emplyee t infrm him/her that the device is being remved. The area supervisr r his designee will infrm the authrized emplyee that the device has been remved prir t the emplyee resuming wrk. 8.0 Testing r Psitining f Machines, Equipment r Cmpnents In situatins in which lckut r tagut devices must be temprarily remved frm the energy islating device and the machine r equipment energized, the fllwing sequence f actins are fllwed: Clear the machine r equipment f tls and materials. Remve emplyees frm the machine r equipment area. Remve the lckut r tagut devices. Energize and prceed with testing r psitining. Deenergize all systems and reapply energy cntrl measures t cntinue the servicing and/r maintenance. 9.0 Outside Persnnel Whenever utside cntractrs are used, the n-site area supervisr r designee

The area supervisr will ensure that that the restrictins and prhibitins f the utside cntractr s Energy Cntrl Prgram are cmmunicated t his/her emplyees Grup Lckut r Tagut When servicing and/r maintenance is perfrmed by a crew, craft, department r ther grup, they utilize a prcedure which affrds the emplyees a level f prtectin equivalent t that prvided by the implementatin f a persnal lckut r tagut device. Primary respnsibility is vested in an authrized emplyee fr a set number f emplyees wrking under the prtectin f a grup lckut r tagut device (such as an peratins lck); The authrized emplyee ascertains the expsure status f individual grup members with regard t the lckut r tagut f the machine r equipment; When mre than ne crew, craft, department, etc. is invlved, the verall jbassciated lckut r tagut cntrl respnsibility will be assigned t an authrized emplyee designated t crdinate affected wrk frces and ensure cntinuity f prtectin; and Each authrized emplyee shall affix a persnal lckut r tagut device t the grup lckut device, grup lckbx, r cmparable mechanism when he r she begins wrk, and shall remve thse devices when he r she stps wrking n the machine r equipment being serviced r maintained Shift r Persnnel Changes Specific prcedures are utilized during shift r persnnel changes t ensure the cntinuity f lckut r tagut prtectin, including the prvisin fr the rderly transfer f lckut r tagut device prtectin between ff-ging and n-cming emplyees, t minimize expsure t hazards frm the unexpected energizatin r start-up f the machine r equipment, r the release f stred energy.
