The fatal crash reported in this document is a sideswipe crash on I 95/SR 9 on the 119th Street overcrossing. According to the police report, vehicle 1 struck vehicle 2. Subsequently, vehicle 1 veered across traffic colliding with vehicle 3 before coming to final rest. This in turn caused vehicle 3 to strike vehicle 4. The driver of vehicle 1 was pronounced dead on scene. After reviewing crash data from , 81 crashes occurred at this location, with 32 of these crashes occurring during dark lighting conditions. A crash diagram depicting the subject crash can be found in this document. Based on review of aerial and street view imagery and crash data, no specific crash trends or roadway characteristics were observed which may have led to the fatal crash documented in this report. 2 STATE OF FLORIDA DEPARTMENT OF TRANSPORTATION DISPOSITION OF FATAL CRASH DISTRICT SIX INITIAL REVIEW Relevant Studies/CTPs R Relevant Programmed Projects N/A Item No : Rigid Pavement Rehabilitation R CRASH NARRATIVE The subject crash occurred on I-95/SR 9 at the NW 119th Street overcrossing in the city of North Miami in Miami-Dade County on February 23, 2013 at 6:16 AM. According to the police report, vehicles 1 and 4 were travelling southbound on I 95 in the right center lane, vehicle 2 was southbound in the outside lane, and vehicle 3 was traveling in the left center lane. The driver of vehicle 1 attempted to the change to the outside lane and collided with vehicle 2 in the front right side. The driver of vehicle 1 over steered and travelled across the left center lane into the inside lane where vehicles 1 and 3 collided. The impact caused the driver of vehicle 1 to lose control of the vehicle and rotate into the express lanes and collide with the center concrete median. Vehicle 3 veered right and collided with vehicle 4 right front corner to left rear passenger door. This impact caused vehicle 4 to rotate and skid through the express lanes and collide with the concrete median wall. The driver of vehicle 1 was pronounced dead on scene. R CRASH DATA REVIEW (crash summary sheets attached) The subject crash was an sideswipe crash. Three years of data (from ) was obtained for I 95 (MP to 9.990) from the FDOT Crash Analysis Reporting (CAR) System. A total of 81 crashes were reported at this location during the three year study period: 33, 26, and 21 for 2009, 2010 and 2011, respectively. Out of the total number of crashes 32 of them occurred during dark lighting conditions. During review of these crashes, one crash was found which involved five pedestrian fatalities. Further investigation showed that this crash was a direct result of an earlier crash which had not yet been cleared, and not a result of roadway conditions. The subject crash is represented in the crash diagram. A review of the high crash segments from the High Crash Roadway Segments Report, obtained from the CAR system, found that the study location has been identified as a high crash segment in 2009 and R OFFICE REVIEW OF FIELD CONDITIONS FDOT video logs and Google Earth aerials were reviewed in the office to identify existing roadway characteristics and conditions that may have led to the fatal crash. From street view imagery, the study segment has 4 southbound lanes and two express southbound lanes. The location has a concrete barrier and paved shoulder on both sides. Plastic delineators are used to separate the standard travel lanes from the express lanes.
