The physical forms are valid for a year from the examination date. It is advisable to arrange physicals well in advance of the sport in which your child is interested in. Please note: Athletic Physical forms are available on the Blevins website or in the Blevins Office. They will not be available in the physician s offices. The athletic fees for are: 7th & 8th grade - $75/sport, Family Maximum - $300/family Athletic and School Insurance Information: PSD insurance does not cover student injuries due to accidents at school or during schoolsponsored activities, such as field trips.
