3 Counselling and Preference form filling Procedure -: Information and Instructions to the Candidates:- 1. Admissions to Medical Postgraduate (MD/MS/Diploma) courses for academic year will be done by counseling & Preference form filling system. 2. Candidates from PGM-CET-2009 State Merit List are called as per following schedule for preference form filling and counselling. Certificates / Documents: Candidate should bring black ballpoint pens, clipboard (Writing Pad), HB pencil and eraser, Information brochure of Preference system and specimen copy of preference form. At the centre, candidates must bring along with them all the relevant original certificates / documents listed below and one set of attested photocopies of the same for scrutiny: a) PGM-CET-2009 Mark-sheet. b) Nationality certificate / valid passport. c) SSC Passing Certificate / valid passport (as proof of age). d) First, Second and Third MBBS mark-sheets (Final part-i & Part-II wherever applicable) e) Attempt certificate of all MBBS examinations from the head of the institution. f) MBBS degree /passing certificate. g) Permanent Registration certificate of Maharashtra Medical Council or other State Medical Councils in India / MCI. h) Certificate from Head of Institute showing that the Medical College/Institute from which the candidates has passed MBBS examination is recognized by Medical Council of India. i) Admit card, Receipt-cum-Identity card. j) Internship completion certificate from University or the Head of the Institution/certificate indicating likely date of internship completion from the Head of the Institution. k) Medical Fitness Certificate from a registered medical practitioner (RMP). (As per Annexure - M of Brochure) If applicable, 1) Caste Certificate. 2) Caste Validity Certificate. 3) Non-Creamy Layer Certificate (for (VJ) DT -A, (NT-1) NT-B, (NT-2) NT-C, (NT-3) NT-D, OBC and SBC Candidates) issued by appropriate authority as specified in the Government Resolution CBC-1094/CR-86/MVK-5, dated 5th June, 1997 and CBC-1094/CR-86/OBC-5, dated 6th June, 2000 and CBC-10/2006/CR-15MVK-5, dated 30th June, 2006 valid upto 31/3/ ) In case of Maharashtra domicile candidate who was selected in 15% AIEE quota and has passed MBBS from institute outside Maharashtra a) 15% AIEE Selection Letter / Letter from Dean for the same b) Certificate of Domicile of Maharashtra state of candidate 5) Disability certificate for Physically Handicapped category claim. Note : 1) Category candidates who do not have Caste Validity Certificate / fresh court order for PG admission regarding Caste Validity, at the time of preference form filling, will be considered as Open merit candidates. 2) DT (VJ), NT-1, NT-2, NT-3, OBC category candidates who do not have Non- Creamy Layer (valid up to 31/03/2009) at the time of preference form filling will be considered as Open merit candidates. 3) Such category candidate without validity and / or NCL, who are considered as Open merit candidate will be issued preference form, if his/her SML Number is within the SML numbers of Open candidates called for Preference Form filling. In case his / her SML number is below the number of Open candidates called for counselling, then he/she will not be eligible for issue of preference form. Information Brochure (2)

5 4) Applicants belonging to SBC: The persons belonging to Special Backward Class (SBC) are to be considered as OBC candidates and such candidates will be required to produce Non-Creamy Layer Certificate. 4. Orientation: Candidates will assemble in respective medical colleges as per the programme. They will be given orientation about the process. Group counseling for Preference form filling will be done. The candidate must be present in person. Proxy will not be allowed. Candidates reporting to wrong centre will not be issued preference form. 5. Preference Form information brochure: This Preference form information brochure is made available to candidates. It contains relevant information about the process, distribution of seats, State Merit List of last admitted candidate for MD / MS / Diploma courses for the academic year 2008, subject & college codes, and specimen of Preference form for practice. Candidates should read the information brochure of Preference system carefully. 6. Scrutiny Form: Scrutiny form will be issued to the candidates. Candidates should fill in required information. They should arrange documents in order as per list given on scrutiny form i.e. one set of originals and another set of attested photocopies. 7. Verification of Documents: Candidates will be called by State Merit List number. They should submit scrutiny form with a set of attested photocopies and show the originals. The documents will be verified by scrutiny officer & eligibility will be endorsed on the scrutiny form. Candidates should not submit original documents. 8. Fees :- i) The fees for Government and Municipal Corporation Medical Colleges, Bombay Hospital and TATA Memorial Hospital, Mumbai is as below (subject to revision by the Govt./ appropriate authorities): Tuition fees Admission fees Rs.18,000/- per year Rs.1,000/- Gymkhana fees Rs. 500/- ii) Hostel fees Rs. 2,000/- per year The following private institutions have communicated to the Competent Authority to make admissions at their institution as permitted by Pravesh Niyantran Samiti. The tuition fees and development fees of these private institutions decided by Shikshan Shulka Samiti (subject to revision by the Shikshan Shulka Samiti) will be as follows: Sr. Name of the Institution Tuition Fee [including development charges per year (Rs.)] 1 Sanjeevan ENT Hospital, Miraj 2,55,000/- 9. Subject & College codes:the subject & college code is a three digit number specific for each subject & college combination. There are such 298 codes. In the preference form even though 315 choices have been provided, a candidate should note that maximum choice available are only 298. Information Brochure (3)

6 10. Subject & College code numbers are specific. Code represents a particular subject in specific college. e.g.:- Code Subject Medical College 101 MD Medicine GMC, Mumbai 110 MD Medicine GMC, Aurangabad 319 DGO GMC, Nagpur 316 DGO BJMC, Pune 617 MS Orthopedics Sancheti Institute, Pune 11. Subject & College codes are given to courses existing in these colleges. For some of these codes the seat(s) may not be available for PGM-CET-2009 Quota during round 1. In the subsequent rounds some seats may become available due to seats surrendered from All India Quota and in-service Quota. These reverted seats will be filled as per the rules of the state quota. Therefore, candidates who desire to opt for these seats can exercise the preference in the preference form for these seats considering eventuality of availability of such seats. 12. Seat distribution chart given in this brochure contains distribution of available seats. The college-wise, subject-wise and category-wise distribution is also available on website. It should be noted that category candidates are eligible and can avail open merit seats by virtue of merit. Category seats, if at all remain unclaimed by a particular category candidate are made available to other categories & to open category candidates as per inter-se category round procedure. Seat distribution chart is subject to change as per instructions of MCI / Transfer of PG Teacher / availability of surrendered seats from All India Quota, In-service Quota etc. 13. All the Course are affiliated to The Maharashtra University Health Sciences, Nashik except M.D. (Anaesthesiology), M.D. (Radiotherapy), D.A., M.D. (Pathology) Post graduate Courses of Tata Memorial Hospital are affiliated to Homi Bhabha National Institute (HBNI) Mumbai (Deemed University) from academic Session a. Physically Handicapped (PH): As per Govt. letter MED-1006 /CR-72 /06 / Edu-2, dated 07/3/2006, three (3%) percent of the total available Postgraduate seats at Government / Corporation Medical Colleges are reserved for the Physically Handicapped candidates. As per MCI guidelines letter MCI-34 (1) / Med / 11773, dated 14/7/2003 the physically handicapped candidates should have locomotory disability of the lower limbs ranging between 50-70% only. The Candidate with a disability more than 70% will not be eligible for admission to postgraduate health sciences courses. As per the MCI guidelines, no other disability shall be allowed. However, such a candidate will be required to undergo medical examination by a special Medical Board constituted for this purpose. Candidate is required to submit the proof of his/her disability by way of a certificate in prescribed proforma from Civil Surgeon of a District Hospital / Medical Board / Medical Superintendent of a Government Medical College / Hospital in Maharashtra; or from the Director of All India Institute of Physical Medicine and Rehabilitation, Haji Ali, Mumbai (as per Prescribed Proforma); at the time of verification of disability. Verification of the disability will be carried out by Special Medical Board constituted for this purpose by Competent Authority. The date and place of Special Medical Board shall be communicated to the concerned candidates telegraphically and by the way of notification on the website i.e. Seat(s) remaining vacant due to non-availability of a Information Brochure (4)

7 Physically Handicapped eligible candidate will be reverted back to the state quota. 14b. Constitutional reservation in private professional educational institutions will be as per the Maharashtra Govt. Gazette notification PGM-1006/3207/CR- 362/06/EDU-2 dated 8 th January, It directs that the seats to be reserved for the candidates belonging to the reserved category to the said courses shall be twenty five percent (25%) of the sanctioned intake. 15. Preference form: Candidates will collect the preference form from the counter. The preference form will be pre printed along with the relevant information of the candidate, which he/she has filled in his/her original application form along with his Roll Number, State Merit List Number, Category Merit Number, PGM-CET Marks etc. Preference form should be handled carefully and should not be folded, stapled or spoilt in any way. 16. Candidate will be issued only ONE Preference Form. Duplicate / another Preference form will not be issued. 17. In case the candidate is not eligible for admission according to rules, the Preference form will not be issued or cancelled if already issued. 18. Candidates remaining absent for the process of preference from filling will not be issued preference form after the programme is over. 19. Candidate should read all instructions before filling the preference form. Candidate should note carefully the subject & college codes. Candidate must first fill the specimen copy of preference form given in the brochure with pencil for practice. Once the candidate is satisfied with his / her choices filled on specimen copy of preference form, then he should start filling the actual preference form using black ball point pen. 20. Candidate should write his most preferred subject and college code as preference no. 1 followed by his next choice as preference no.2 and so on. While filling the subject and college codes, number should be written in boxes, one digit in each box (3 digit number - 3 boxes). As the form will be scanned by Computer, candidate should write the codes with black ball point pen and with neat, legible, clear handwriting. 21. Candidate must be very careful while filling the preference form. Candidate can fill minimum one preference or any desired number of preferences or maximum 298 preferences. The same code should not be filled more than once. In case the candidate fills the same code more than once, higher preference will be considered and subsequent repeated preference(s) will be ignored. If the candidate fills wrong code number that preference will not be considered. 22. If a candidate does not give preference for a particular code (Govt. / Corporation / Private); then it means that he / she is not interested / desiring to take that PG seat. Candidate should note that there will be no change / addition / deletion in his / her list of preferences at later date / stage (not applicable for final round by personal appearance). 23. The candidate should submit his / her duly filled Preference form at the specified counter. Candidate shall inform regarding any correction if required at the counter. 24. Data Response Sheet & Verification: The preference form will be scanned and data response sheet in duplicate will be generated and will be given to the candidate for checking. Candidate should verify that the data i.e. SML Name, Category etc. and his preferences and subject college codes have been correctly printed. After verification the candidate can report discrepancy if any in the data and it will be duly corrected. If the candidate does not report any Information Brochure (5)

8 discrepancy, it will be presumed that the data entered against his / her name is correct and subsequently corrections will not be made at later date. Then the candidate should sign the data response sheet. 25. Candidate should note that after verification by the candidate, the preferences exercised by the candidate shall be final and irrevocable. Thereafter there can not be any change in the preferences. This final data shall be used in the entire selection process for admission to Medical Postgraduate courses (except for final round by personal appearance) 26. If a candidate gets selected for a particular PG course, then the candidate is required to join that course as per the procedure. In case he / she does not join the course, he / she will not be eligible for subsequent admission process of PGM-CET For filling of vacant seats after first round, same preferences will be considered and no separate preference form will be required to be filled by the candidate (except for final round by personal appearance). 28. Final round (By Personal Counseling): A) There shall be a final round for allotment of seats remaining vacant after completion of the earlier rounds of allotment of seats by way of preference forms. This final round of allotment of seats shall be conducted by way of personal counseling instead of allotment on the basis of preference forms. On account of allotment of seats in this final round of admission, no fresh vacancies consequent upon allotments made in this round will be created. This is principally because this final round of allotment is meant to fill in (as far as possible all) available vacant seats without creating a new or fresh vacant seat on account of such allotment. At this round, eligible candidates will appear personally, seeking allotment of seats of their choice. The personal counselling for such allotment will be held strictly in the order of merit. Each candidate will be offered all seats that have remain vacant and are available for allotment, only at the time of his/her turn of counselling i.e. State Merit List position. B) The following candidates will not be eligible for being considered for allotment of seat in this round:- I. All PGM-CET-2009 candidates to whom a seat has been allotted in 1st round but have not joined or those who have joined and resigned are not eligible for personal counseling round. II. All the candidates to whom a seat has been allotted in the 1st round and who have joined the college accordingly and have further submitted Status Retention Forms by the aforesaid prescribed date will be removed out of consideration for the remaining admission process. III. PGM-CET-2009 candidate who have been allotted a seat thorough All India Postgraduate quota in Medical Colleges in Maharashtra and joined the College and have not surrendered the seat to the State quota by giving an application before the prescribed date. C) It is hereby clarified that all such candidates to whom no allotment of any seat was made in the previous rounds of this admission process, shall be expressly eligible for being considered for an allotment of a seat in this round. 9 D) The preferences given by all the candidates who are eligible for being considered for allotment of seats in this round by way of preference forms submitted in this admission process will stand cancelled and the allotment shall be made in this round without reference to such preferences on the basis of choice made on the spot by the candidate at the time of personal appearance counseling. E) All the candidates to whom the seats will be allotted in accordance with the choice and available seats at his/her merit position at the time of personal counseling, during the course of this round will be required to join the concerned college and complete all necessary formalities requisite for taking admission accordingly within the prescribed time. F) Allotment of seat once made in this personal counseling round, shall not be changed and/or cancelled at any count whatsoever. Therefore, while giving the choice on the spot at the time of this personal counseling round, the candidates must consider the available seats thoroughly and opt for a seat which they must join once allotted. It is made hereby clear that in case such a candidate does not join the concerned College and/or fails to comply with the necessary formalities of the admission within the prescribed time, such a candidate shall not be eligible for appearing at immediately next succeeding PGM-CET conducted for grant of admissions to postgraduate courses in the State of Maharashtra i.e. PGM CET 2010 and will have to pay Rs.1,00,000/- as penalty for lapse of post graduation medical seat. G) Final round by personal appearance will be held only in Mumbai. Note : In this process of preference form filling and subsequent round/s (If any), admissions to those post graduate courses will be done which are recognized, recommended for recognition and unrecognized by Medical Council of India. Hence the candidate are instructed to fill the preferences for all such courses accordingly, if they desired so. How ever in case of unrecognized courses the candidate will have to submit an under taking at the time of preference form filling and also at the time of admission at the respective Institute in the prescribed format. * * * * * * * * * * Information Brochure (7)

10 SELECTION PROCESS 1. The collected data will be centrally processed for the selection of candidates for MD/MS/Diploma courses. 2. The selection will be done as per merit (SML no.), category and on the basis of Preferences given by candidates in the Preference form. 3. The list of candidates selected for MD/MS/Diploma courses shall be displayed at the website of DMER i.e. and also at the office of Government Medical Colleges, Mumbai/Pune/Nagpur/Aurangabad. 4. The selection list will be declared by 15 th April It will be displayed on the website and the offices of above four centers. List of selected candidates will be sent to all concerned colleges. Candidate should report and pay the fees and submit original certificates at respective colleges by 24 th April No separate selection letter will be issued to candidates. 5. If the candidate does not report by specified date, then his / her selection automatically stands cancelled and he / she will not be eligible for subsequent process. 6. Candidate who is selected for PGM-CET-2009 State Quota seat and also for All India 2009 Quota seat will have to decide to opt for one of them. In case he wants to take State Quota Seat, he has to cancel his All India seat by giving application to the concerned Dean and then join the PGM-CET-2009 State Quota seat by giving joining report, submitting original certificates and paying fees, within stipulated joining period. 7. The academic term of Medical Postgraduate courses shall start from 2 nd May, and residency program will also start from 2 nd May Process for filling vacant seats: After first round, the vacant seats and seats surrendered from All India Quota, if any will be filled in. For these seats the selected as well as not selected candidates who have filled the Preference form are eligible. For this process PGM-CET-2009 merit list of eligible candidates will be operated from SML number 1 onwards. The candidate getting selected in round one will be considered for betterment in the subsequent selection process. The betterment herein means the higher Preference exercised by the candidate. The shift in such betterment shall be compulsory and mandatory except for those who have filled 'Status Retention Form.' Such a candidate who has filled Status Retention form will not be considered for any subsequent rounds of selection process. The last date for filling of Status Retention Form will be notified in the notification of the selection round. However those candidates opting for betterment will have to collect original certificates and admissible refund of fees as per procedure from previous college and join the new course and college within stipulated joining period. 9. Candidate who has confirmed his/her admission may cancel it by submitting an application to the concerned Dean. 10. Candidates may resign before prescribed dates which will be notified due course of time to be eligible for PGM-CET However such a candidate is eligible for subsequent admission process (PGM-CET-2009) for the final round by personal appearance. 11. Candidates canceling his/her admission after prescribed date resulting in lapse of a seat will not be eligible for the PGM-CET The procedure for refund of fees shall be as under. a) On or before prescribed date Rs. 1000/- to be deducted from the tuition fees and rest to be refunded. b) After prescribed date No refund 13.
