5 Preparation before filling in questionnaire :Select comment mark from Options Setting for viewing comments In this questionnaire, there are comments which belong to necessary cell for facilitating to fill out the form. When you open EXCEL file, Please always check the applicable point of the comment dialogue box as follows. "tool (T) " " option (O) " " View " " comment " Select " comment mark" Set comment marks only, then Touch the mark of red triangle, then the comment is appeared. 5

6 Procedure1 Confirmation of Structure of Chemical Substance Examination Sheet You need to fill out Item list *1 and Examination sheet *2. And data sheet *3 (ICP data,msds) if required. Choose either wt% Ver or Mass Ver in Examination sheet *2. Refer to the selection criteria on page 11. In case of copy sheets from the Examination Sheet, Be sure to follow below. Edit Move or Copy Sheet (M) on the menu bar of Excel Sheet. If you need to attach ICP data or MSDS. * 3 Item list *1 Choose either wt% Ver or Mass Ver in Examination sheet. *2 6

8 Procedure2 Supplementary Explanation-1 : Existence of SONY Parts Number Sony parts Number Exist or not If you choose Yes, the option will be started. Then enter SONY Parts Number into the column. *If not clear, choose No. 8

9 Procedure2 Supplementary Explanation-2 Group name 1 Items are composed by same chemical substance s name and content ratio, put those items together to one group. Can be summarized several Examination Sheets to one Exam. Sheet using the Exam. Sheet Percent (wt%) Version in case where the same group name. 2Group name is the key word connects Item list with Examination Sheet. Hence, enter the item group name into the column Group name even if one item. *Group name of same Manufacturer s Item name should be tallied for each other on Item List. * The Group Name on Examination Sheet [Item List] should be same as the Group Name on Examination Sheet [wt% Version] and [Mass Version]. 9

For example: Combination of EE type ferrite core, male connector and female connector. -Please input mass per unit. Available unit should be selected from 6 units. -In case of connected PCB, input mass per a piece of PCB after divided. -Please confirm the cell color turning to yellow from pink after entering mass. The pinkness is NG 10

11 Procedure3 Selection of Examination Sheet Refer to the table below, and choose Examination Sheet for entry. Percent (wt%) Version :Amount of contained chemicals are shown by content ratio. Mass Version : Amount of contained chemicals are shown by total mass. =>The merit of selection for each version are highlighted by yellow color in the table below. (Common item for Percent Version and Mass Version ) Select proper type from For chemicals, For parts or For packaging Necessary entry items will be shown by selected type. 2 Select either Conformity or Nonconformity to TDK Green Procurement Standard. 3 For For chemicals or For parts, select Conformity or Nonconformity to RoHS Directive. For For packaging, select Conformity or Nonconformity to Packaging Waste Directive. 4 Purchasing products are already applicable for Sony specification raw materials, select Applicable. (Refer to P20) 5 In case where data can not be disclosed, select Not disclose all or Not disclose partially. 6 If apply RoHS Directive or Packaging Waste Directive, select Yes. If not apply, select No 7 If contain impurities clearly, select "Yes

(Refer to next page) 2 Enter Subpart (Homogeneous material) (Refer to next page) 3 TDK Chemical Substance Classification Number will automatically be listed when you select 4 Class of chemicals. 4 Select Chemical Substances. If not applicable, choose Other substances. 5 After select Chemical Substances at the step of No.4, enter the name of chemical substances if the color of cell turns pink. (Refer to the figure below) 6 The cell color turns pink, enter CAS Number into this cell. *Make sure to fill out into all cells, also pink ones. (Refer to the figure below) Be sure to enter CAS Number, except substances which have no Number. 4 As touching the cell for chemical substances by your cursor, you can select an applicable material name from list of materials on the drop-down menu. a. If there is no applicable material in the list of materials on the drop-down menu, be sure to select "other component (X00000)". In this case, be sure to fill out the detail material name into "5 chemical substance column". b. In case of selecting "other ****compounds", be sure to fill out the detail material name into "5 chemical substance column". 5 In case of materials which are undisclosed or unknown, be sure to select "other component (X00000) and fill out material name into "5 chemical substance column" accordance with examples below. "material A, material B, material C or material 1, material 2, material 3..." Only if selecting "other components" in the list of materials, strictly inhibited to select "others" in "5 chemical substance column". Please refer to "list of materials" in the EXCEL sheet if you need to confirm "material list". 14

(Common item for Percent Version and Mass Version )-5 In case error message is indicated in spite of correct number, configuration of Excel sheets might has changed. The causes of change are as below. Would you please reconfirm your operation? 1. When you save the Excel file which have some selected worksheets. 2. When there is a numerical formula of circular reference on worksheet. 3. When you open Excel file as above. <Applicable to Microsoft office Excel before edition 2003.> Corrective action Menu bar: "Tool" => "Option" => "Calculation" => Select "Automatic" Refer to attached image. 16

(Common item for Percent Version and Mass Version )-4 The column of Content total(%) For For parts and For packaging, Content total should be 100% in principle. For For chemicals, Content total (%) should be exceeded 100%. Every type include parts of non disclosure is not allowed below 100%. 18

19 Procedure 4 Exam. Sheet. Make an entry of detailed information. (Common item for Percent Version and Mass Version )-5 Percent Version a. Content :Enter weight percent of each subpart to total. (Refer to column a ) b. Content Subpart : Enter weight percent of each chemical substance to the subpart into a cell for each chemical substance. (Refer to column b ) When the value of content ratio is below criteria, if enter the rounded value as same value of criteria into Examination sheet, it becomes non conformity. Therefore, not to round the value of content ratio to be as same as value of criteria. Ex. Criteria of existing Cadmium in Lead; below 0.002wt% In case the investigation result is wt%, be sure to enter wt%, never to enter 0.002wt% Mass Version c. Subpart Total Mass : Enter mass (mg) of each subpart into a cell for each subpart. (Refer to column c ) d. Mass : Enter mass (mg) of each chemical substance into a cell for each chemical substance. Total mass of chemical substances should be tallied with mass of the corresponded subpart. (Refer to column d ) a b c d TOTAL 100% Total mass of chemical substances should be tallied with mass of the corresponded subpart. (In this case, total 0.3mg) Content of each subpart to total. And also any other comment can write down in this column

(Common item for Percent Version and Mass Version )-8 If purchase raw material from manufacturers are designated by SONY as Green-partner, choose Applicable. Choose Applicable at the column of SONY specification raw material, then the option will be started. 12 Choose Applicable, if Resin, Coated wires covered by resin by SONY are used. 13 Enter the ID Number given to the material maker. 14 Choose Exist, if the material is listed on Green book. 15 Enter the Registered Number on Green book. 16 In case where No.12 is applicable and No. 13,14,15 are not disclosed, if the raw material is purchased from Green partner, choose Yes * Check these values in each subpart No need to check every chemical substances in each subpart. 22

24 Prohibited Items:To delete sheets It is strictly prohibited to delete any sheets from Content Chemical Substances Examination Sheet. And also strictly prohibited to move or to copy. 24

25 Prohibited Items:To enter the multiple name of purchased items into Chemical Substances list. It is strictly prohibited to enter multiple name of purchased items into Chemical Substances list In this case, make a sheet copy for each purchased items and enter manufacturer s item name. Such as products which is composed by multiple parts like a cable kit is not applicable in this case. In such a case, enter all parts name per one product into a sheet. 25

26 Prohibited Items:To enter other characters except numerical value into the cell for content rate and weight. Only numerical value can be entered into the cell for content rate and weight. Non-numerical value is strictly prohibited. (Ex:~,, below, less etc.) Examination Sheet ( Wt% Ver or Mass Ver) 26

27 Prohibited Items: Submitting using other application software except Microsoft Excel Be sure not to use other application software except Microsoft office Excel when generate Content Chemical Substances Examination Sheet. (For example PDF file / Free Open Office soft etc.) 27
