Registration certificate in the A.P. nursing counsel both Nursing & Midwifery. 6. Caste Certificate (incase of SC/ST/BC issued by the Revenue authorities). 7. Study Certificates from 4 th to 10 th class. In case of Private study Residence certificate to the relevant period (for seven years) issued by the Mandal Revenue Officer / Tahsildar concerned in the prescribe Proforma). 8. Physically handicapped certificate issued by the Regional Medical Board / Sadaram incase of physically handicapped persons (If applicable). 9. Work experience certificate in 300 hundred bedded multi Specialty hospital for weight age marks. She / He is informed that No T.A. and D.A. will be paid for attending Interview. She / He should make their own arrangements to stay for the above dates. The candidate is also informed that if She / He does not attend on the above dates, She / He has to forego the opportunity for appointment on contract basis and will not be considered at later date.
