1 EU 向けに輸出される食品等に関する証明書の発行に係る事務処理要領 平成 23 年 4 月 7 日山口県農林水産部流通企画室 ( 趣旨 ) 第 1 平成 23 年 3 月 27 日付け22 国際第 1144 号農林水産省大臣官房総括審議官 ( 国際 ) 通知に基づき 欧州連合 ( 以下 EU という ) へ輸出される食品及び飼料の証明書の発行条件及び手続きを定めるものとする ( 対象となる食品等 ) 第 2 証明書発行の対象となる食品等は 日本で産出され 又は 日本から発送される食品 ( 直接又は加工後に食されることを意図した産品 ) 及び飼料 ( 動物の餌とすることを専ら目的とした産品 ) であって水産物以外のものとする ( 発行要件 ) 第 3 次のいずれかの要件を満たす食品等に証明書を発行することとする 1 山口県内で 平成 23 年 3 月 11 日より前に 生鮮食品にあっては収穫 加工食品にあっては最終加工されたものであること ( 日付証明 ) 2 平成 23 年 3 月 11 日以降で 生鮮食品については 山口県内で収穫したものであること ( 産地証明 ) 3 平成 23 年 3 月 11 日以降で 加工食品については 山口県内で最終加工されたものであること ( 産地証明 ) ( 申請手続き ) 第 4 証明書の発行を申請する者は 次の 1 から 6 に掲げる書類を県に提出するものとする 1 証明申請書 ( 別記様式 1 号 ) 2 EU への輸出証明書 ( 別記様式 2 号 ) 3 第 3 の 1 に該当する場合は 収穫又は加工の製造年月日を証明することができる書類 4 第 3 の 2 又は 3 に該当する場合は 収穫又は加工された産出地を証明することができる書類及び主原料の生産地を証明することができる書類 主原料の生産地が福島県 群馬県 茨城県 栃木県 宮城県 山梨県 埼玉県 東京都 千葉県 神奈川県 静岡県及び岩手県の場合にあっては 公的な検査機関による放射能検査の結果により安全性を示す書類が必要 5 その他 EU への輸出証明書記載事項を確認することができる書類 6 郵送による受取を希望する場合は 住所等を記入し 切手を貼付した返信用封筒
