1 Revision Checklist Through Revision 26 INSERT REVISION 26 FIRST - THEN USE THIS CHECKLIST INSTRUCTIONS 1. Go through the Guide page by page comparing the CHANGE date (bottom, right of each page) with the CHANGE date on the checklist. Mark the checklist to show any pages you are missing, or pages with CHANGE dates earlier than that shown on the checklist. 2. Pages in your Guide that are NOT on the checklist should be discarded. 3. If the checklist shows that you are missing all pages for either Revision 23, 24, 25 or 26 we will replace one missing revision free of charge. 4. In addition to replacing one missing revision, we will also replace up to 16 individual sheets free of charge. However, if the checklist shows that you need more than 16 individual sheets, a complete set of new contents, priced at $18.00 will be required. 5. Record the pages (or revision) you need in the space provided on page 2 of this checklist. Please use complete page number and abbreviated airport name(s) to assure receiving the correct sheets. 6.
