4 Directory Listing 12.1 INRODUCION 1. he Directory Listing (DL) Form is prepared by the customer and is forwarded to Sprint LD. his form provides the administrative, listing and directory delivery information associated with the customer's request. It is always associated with an LSR Form. See General Information at the beginning of this document for appropriate request type and activity combinations. 2. All information required for ordering listings is provided in the various fields contained within the DL Form. he Listing Control Section provides entries for the type of activity and listing involved, the type of account for which the listing is being requested, etc. he Listing Instruction Section provides specific listing details. he Delivery Section contains information pertaining to the quantity and address for directory delivery. he Advertising Section contains information for the Yellow Pages. 3. When Activity is W, Sprint LD will migrate all listings associated with the ANs for a request AS IS. 4.
