Facilities Management Plicy Number: 700.11 Title: Chemical Hygiene Plan Implementatin Date: 2002 Last Audit: May, 2018 Last Revised: June 20 th, 2018 Intrductin The U.S. Department f Labr and the Occupatinal Safety and Health Administratin (OSHA) prmulgated the final rule (29 CFR 1910.1450) titled Occupatinal Expsures t Hazardus Chemicals in Labratries n 31 January 1990. Tennessee Occupatinal Safety and Health Administratin fficials have adpted this standard verbatim. This new standard differs frm many OSHA health standards in that it des nt establish new expsure limits, but sets perfrmance prvisins designed t prtect labratry wrkers frm ptential hazards in their wrk envirnment. Purpse The primary purpse f the East Tennessee State University Chemical Hygiene Plan (CHP) is t prtect emplyees and t reduce the risk f injury frm chemical hazards assciated with particular labratries. This is accmplished by establishing respnsibilities, plicies and prcedures fr handling hazardus chemicals and thrugh the develpment and implementatin f wrk practices and cntrl measures expressly tailred t the varius labratries present at the University. Additinally this plan serves as a guide fr the varius University Divisins as they develp their specific Chemical Hygiene Plans. Definitins Chemical Hygiene Officer - The designated emplyee wh is qualified by training r experience t prvide technical guidance in the develpment and implementatin f the Chemical Hygiene Plan. Chemical Hygiene Plan - A written prgram which sets frth plicy and prcedures t prtect emplyees frm the health hazards assciated with their wrk place. Cmbustible liquid - Any liquid having a flashpint at r abve 100 degrees Fahrenheit (F). Cmpressed gas - A gas r mixture f gases having, in a cntainer, an abslute pressure exceeding 40 psi at 70 degrees F, r a gas r mixture f gases having, in a cntainer, an abslute pressure exceeding 104 psi at 130 degrees F regardless f the pressure at 70 degrees F, r a liquid having a vapr pressure exceeding 40 psi at 100 degrees F as determined by ASTM D-323-72. Emplyee - An individual emplyed t wrk in a labratry wh may be expsed t hazardus chemicals in the curse f their emplyment. Explsive - A chemical that causes a sudden, almst instantaneus release f pressure, gas and heat when subjected t sudden shck, pressure r high temperature. Flammable liquid - A liquid having a flash pint belw 100 degree F, except any mixture having cmpnents with flash pints f 100 F r higher, the ttal f which make up 99 percent r mre f the ttal vlume f the mixture. Flammable slid - A slid ther than a blasting agent r explsive that is liable t cause fire thrugh frictin, absrptin f misture, spntaneus chemical change, r retained heat frm manufacturing r prcessing, r which can be ignited readily and when ignited burns vigrusly and persistently as t create a serius hazard. A chemical that ignites and burns with a self-sustained flame at a rate greater than ne-tenth f an inch per secnd alng its majr axis when tested by the methd described in 16 CFR 1500.44. Glve bx- A sealed, prtectively lined cmpartment, having prts t which are attached glves fr use in handling materials inside the cmpartment Hazardus chemical - A chemical fr which there is statistically significant evidence based n at least ne study cnducted in accrdance with established scientific principles that acute r chrnic health effects may ccur in an expsed emplyee. This includes chemicals which are carcingens, txic r highly txic agents, reprductive txins, irritants, crrsives, sensitizers, hepattxins, nephrtxins, neurtxins, agents which act n the hematpietic (bld-frming) systems, and agents which can damage the lungs, skin, eyes r mucus membranes. High Risk Operatins - Experimental prcedures invlving the manipulatin, handling r reactin f hazardus chemicals where the ptential fr release f gas, vapr r aersl cntaminatin is high. This categry includes but is nt limited t (i) rapid exthermic reactins, (ii) transfer f electrstatic pwders, (iii) heating, mixing r transfer f vlatile chemicals, (iv) pressurized peratins where there is ptential fr uncntrlled release, and (v) wrk invlving aersl generatin. Labratry - A facility r individual rm where the "labratry use" f hazardus chemicals ccurs. Labratry hd - A type f engineering cntrl enclsed n five sides with a mvable sash r fixed partial enclsure n the remaining side designed t draw air frm the labratry int the enclsure t prevent r minimize the escape f cntaminants int the labratry space. Labratry scale - Wrk with substances in which the equipment used fr reactins, transfers, and ther handling are designed t be easily and safely manipulated by ne persn. Oxidizer - A chemical ther than a blasting agent r explsive as defined in Title 29 CFR, part 1910.109 (a), that initiates r prmtes cmbustin in ther materials, thereby causing fire either f itself r thrugh the release f xygen r ther gases. Universal Precautins Universal precautins is an apprach t infectin cntrl t treat all human bld and certain human bdy fluids as if they were knwn t be infectius fr HIV, HBV and ther bldbrne pathgens. Respnsibilities Envirnmental Health and Safety Office (EH&S) Develp and maintain this written university wide plan. Mnitr department cmpliance with the prgram by annually reviewing the chemical hygiene plans f each applicable department. Prvide guidance and technical assistance t departments in the implementatin and maintenance f the prgram. Assist departments in fulfilling their training requirements. Prvide guidance and assistance with hazardus waste handling, strage and dispsal. Apprve departmental prcurement f chemicals t ensure that the apprpriate engineering cntrls are available fr the chemicals being purchased thrugh ebucs. Departments that Have Labratries Appint a Chemical Hygiene Officer (OSHA Crdinatr) t facilitate implementatin f this Prgram. Ensure all necessary persnal prtective equipment has been prvided. Ensure necessary and required training is prvided t ptentially expsed emplyees. Mnitr and enfrce cmpliance with Universal Precautins. Departmental Chemical Hygiene Officers Perfrm surveys t insure labratries are in cmpliance with this prgram. Ensure new emplyees are riented t this standard when initially hired. Maintain all departmental recrds required by the prgram. Review the Departmental Chemical Hygiene Plan annually and revise as needed. Review must be dcumented in writing. Dcumentatin may be as simple as writing and signing a statement n the cver page stating that the annual review has been perfrmed. Ensure apprpriate persnal prtective equipment is wrn by all labratry persnnel and visitrs. Ensure all Hazardus Waste cntainers are apprpriately labeled. Emplyees, Students and Other Ptentially Expsed Individuals Understand and cmply with the prvisins f this Prgram and the prtectin affrded by the OSHA standard. Ntify yur departmental, OSHA Crdinatr r ther University fficial f activities, which present ptential expsure cncerns. Be aware f engineering cntrls and the prper use f thse cntrls. Fllw established cntrls t eliminate r minimize ptential expsure. Be aware f the prper use, limitatins and lcatin f available persnal prtective equipment. Use apprpriate persnal prtective equipment t eliminate r minimize ptential expsure. Be aware f and bserve established husekeeping prcedures, e.g. use f mechanical devices t clean up brken glass in lieu f using bare hands. Maintain wrk area in a clean and sanitary manner. Methds f Cmpliance Prcurement. Purchase requests fr chemicals shall include a request fr the supplier t prvide a cpy f any applicable SDS t the EH&S Office. Persnnel shuld rder the smallest quantity necessary t cmplete the wrk. Persnnel wh initiate purchase requests shuld review health and safety data n chemicals prir t rdering t determine any special requirements fr handling, strage r dispsal. Safety Data Sheets (SDS) fr chemicals used at ETSU are available thrugh the Health and Safety Office r thrugh MSDS Online. Cntainers shuld be inspected upn receipt t ensure they are intact and nt leaking. Damaged r unlabeled cntainers shall nt be accepted. Chemical Strage. Chemical strage inside the labratry shuld be limited t thse chemicals necessary fr wrk in prgress. Central strerms shall be used when they are available. Chemicals shuld nt be stred n the bench. Open shelves shuld be designed with a restraining device r lip t prevent cntainers frm creeping r tipping ver. Chemicals shall be stred accrding t the cmpatibility categries. Chemicals within a given strage grup may be incmpatible with ther chemicals in that grup. Spill trays shuld be used t reduce cmmingling in the event f spills r leaks. Chemicals shall be inspected at least semiannually t determine their cnditin. Crrded r leaking cntainers shuld be turned in as hazardus waste. Cabinets and strage areas shall be labeled. MSDS Online Prgram. The MSDS Online Prgram is managed by ETSU s Envirnmental Health & Safety Office (EH&S). EH&S will add all new r updated Safety Data Sheets (SDS) t the database as each department purchases new chemicals. This prgram is available 24 hurs a day, 7 days a week and can be easily accessed when the internet is available. Hard cpies are available thrugh EH&S at 439-6028. Additinally, the university s prcurement system autmatically flags chemical purchases. N chemicals are apprved thrugh this prcurement system withut review/apprval by EH&S. Flammable and Cmbustible Liquids. If the lab has a sprinkler system, n mre than 5 gallns f flammable and cmbustible liquids, cmbined, shuld be stred utside f a flammable strage cabinet. If the lab des nt have a sprinkler system, n mre than 2 gallns shuld be stred utside f a flammable strage cabinet. Flammable and cmbustible liquids shall be stred in glass, metal r plastic cntainers which meet the requirements f NFPA 30. Flammable liquids shall be stred in apprved safety cans when the cntainer quantity exceeds 2 gallns. Cmbustible liquids shall be stred in apprved safety cans when the cntainer quantity exceeds 5 gallns (NFPA 45). Flammable and cmbustible liquids shall be stred in apprved cabinets designed in accrdance with NFPA 30. Cabinets shuld nt be lcated adjacent t an exit r in a stairwell. The transfer f flammable liquids t smaller cntainers frm bulk cntainers nt exceeding 5 gallns shall be cnducted in a chemical fume hd r in an apprved inside chemical strage rm. Flammable liquids shall nt be transferred between metal cntainers unless the cntainers are electrically bnded. Refrigeratrs and freezers used t stre flammable liquids shall be explsin-prf r "labratry safe" in accrdance with NFPA 45. Water Reactive and Shck Sensitive Chemicals. Water reactive chemicals shall be segregated frm ther chemical strage. These chemicals shuld be stred in apprved cabinets. If apprved cabinets are nt available, cntainers shuld be ver packed in a metal can during strage. Water reactive chemicals shall nt be stred with flammable r cmbustible liquids. Shck sensitive and perxide frming chemicals, if expired, shuld be dispsed f by EH&S persnnel. Plicy Number 700.11 Chemical Hygiene Plan Page 6 f 9

Cmpressed Gases. Gas cylinders shall be labeled r tagged t shw their cntents. Gas cylinders shall be secured by the use f clamps, chains, straps, r therwise restrained while in strage r use. When gas cylinders are in strage, hand valves shall be tightly clsed and the valve prtectr cap shall be in place. Cmpressed gas frm cylinders shall be reduced thrugh the use f a regulatr specifically designed fr that purpse. Reductin valves, gauges and fittings used fr xygen shall nt be used fr ther gases. Likewise valves, gauges and fittings used fr ther gases shall nt be used fr xygen. Cmpressed gas cylinders shall be mved using a suitable hand truck. The gas cylinder shall be strapped in place with the valve prtectr cap installed. Only ne cylinder shall be mved at a time. Transprting Chemicals. Txic, flammable r crrsive chemicals shuld be placed in a carrying bucket r ther unbreakable cntainer when mved between rms r thrugh the labratry crridrs. Wheeled carts shuld be used t mve larger quantities f chemicals which cannt be hand carried. Carts with pen shelves shuld be designed with a restraining device r lip t prevent cntainers frm creeping r tipping ver. Engineering Cntrls. Engineering cntrls including hds, glve bxes, inhalatin chambers, gas cabinets, lcal exhaust ventilatin and substitutin f less txic chemicals shuld be used t minimize expsure t all hazardus chemicals in the labratry. Labratry peratins shall be planned and cnducted using apprpriate engineering cntrls. High risk peratins shall be cnducted inside primary cntainment including chemical hds, glve bxes r inhalatin chambers. Plicy Number 700.11 Chemical Hygiene Plan Page 7 f 9

Chemical Exhaust Hds. Hds shall have an average face velcity f 90 feet per minute (fpm) r greater with the sash in the full pen psitin. Hds used fr radiactive materials shall have an average face velcity f 100 feet per minute (fpm) r greater. Sash stps shuld be installed when the face velcity requirement cannt be met with the sash in the full pen psitin. Hd perfrmance will be evaluated annually by EH&S and after any repair r mdificatin. Glve bxes. Glve bxes shall be visually inspected prir t any wrk. Maximum life f glves are 10 years frm the dcumented date stamped frm the manufacturer. If glves are changed ut fr any reasn, it must be dcumented (date, manufacturer and mdel, and the persn ding the change ut). Glve bx perfrmance shall be evaluated annually, and after any repair r mdificatin t the ventilatin system. Administrative & Wrk Practice Cntrls. Wrking quantities f hazardus chemicals utside f strage during an peratin shall be as small as practical. Cntainers shall be clsed when nt in use. Muth pipetting is prhibited. Handle and stre labratry glassware with care t avid damage. Damaged glassware shuld nt be used. Glassware used fr pressure r vacuum service shall be designed specifically fr that purpse. Wrk with the hd sash clsed as much as pssible during the peratin. Keep all apparatus and cntainers at least 6 inches behind the hd face t minimize spillage frm the hd. Minimize the strage f chemicals r hazardus waste inside the hd. Use apprved cabinet r satellite strage lcatins. Plicy Number 700.11 Chemical Hygiene Plan Page 8 f 9

If the hd sash is suppsed t be partially clsed fr peratin, the hd shuld be s labeled and the apprpriate clsure pint clearly marked. Persnal Prtective Equipment PPE shall be prvided, laundered r dispsed f at n cst t the emplyee. All persnal prtective equipment shall be repaired r replaced as needed. All persnal prtective clthing shall be remved prir t leaving the wrk area. Dispsable glves shall be replaced as sn as practical when cntaminated r as sn as feasible if they are trn, punctured r when their ability t functin as a barrier is cmprmised. Glves shall be f apprpriate materials, intact latex r intact vinyl f apprpriate quality fr the prcedures perfrmed and f apprpriate size fr each wearer. Hands are t be washed using warm water and liquid sap immediately after remving glves. N glves shall be used if they are peeling, cracking r disclred f if they have punctures, tears r ther evidence f deteriratin.
