1.Purpose The essential points to chemistry substance content investigation based on a Standard document are defined. 2.Survey Scope The survey covers electrical and electronic devices and their component parts, materials, and accessories. Packaging materials are excluded in principle, but surveys may be required according to the needs of the surveying company. However, packaging materials that the surveyed company uses for transporting and protecting products sold to the surveying company shall be excluded. 2

3.Survey Items (1) Basic Information about company (a) Reference Number This number is used to manage each survey file at the surveying company, and you do not need to fill this in, in principle. However, please do so when instructed by the surveying company. (b) Date of Data Entry The surveying company enters the date the survey is requested. Do not enter or change this date. (c) Surveying Company This is the information about the surveying company. The column boxes 1 to 3 are used as set by the surveying company. Do not make entries or changes here. (d) Response Date Enter the date you will send in the survey. (e) Surveyed Company Enter your company name, DUNS number, address, division, name, contact name, telephone number, fax number, and email address. If you are a trading company, enter your information rather than the manufacturer information. Fill out columns 4 to 6 based on the instructions of the requesting company. Otherwise, do not make entries or changes here. Note: The DUNS number is a unique nine-digit identification code assigned by the Dun & Bradstreet Corporation. Leave it blank if you do not know your DUNS number. (2)Information about Parts / Products / Material (f) Parts Number (used at surveying company), Parts Name, and Surveying Company Columns 1 to 3 The part number and name are established by the Surveying Company for the survey item. In principle, the Parts Number., Parts Name, and Surveying Company Columns 1 to 3 are completed by the Surveying Company, however follow any instructions from the surveying company. (g) Manufacturer s Name, Parts Number (used at surveyed company) Enter the manufacturer name and part number for the survey item. (h) Investigation unit Select the unit for the survey item when entering the amount contained from the pull-down menu. However, if the Surveying Company has set the unit already, follow the Surveying Company s instructions.e.g. Choose "units" for parts, in principle, and for raw materials choose 3

the most suitable unit from kilograms, square centimeters, square meters, cubic meters, meters, liters, or grams (i) Parts Mass Enter the total weight of the part per the unit set in (h) above, using the specified unit, "grams. " E.g. :If the survey unit is units Æ the weight in "grams" per one survey item unit :If the survey unit is kilograms Æ the weight in "1000 grams" per kilogram (j) Composition information Please input a composition part, a manufacture maker name, and partial weight. The name of a composition portion should answer by the name shown on specifications and the drawing, and the general name. Please input part weight by mg. (k) Material norms / standards Please enter numbers of material norms / standards. Ex. enter ISO1043 for resin, ISO1629 for rubber, and ISO18064 for thermoplastic elastomers. (l) Material numbers / symbols For resin / rubber, please enter the material symbol. Ex. enter material symbols defined by ISO1043 for resin, ISO1629 for rubber, and ISO18064 for thermoplastic elastomers. For metal or materials other than resin and rubber, please enter material symbols specified by official material norms / standards. Ex. enter S45C for Steel S45C. Please enter "EP-GF30" for epoxy resin filled by glass fiber at 30% (Plastics EP). (m) Use of Ozone-depleting Substances in Manufacturing Process Indicate whether or not ozone-layer depleting substances are used in the manufacturing process (see Attachment 4 Breakdown Substances). Indicate such substances even if they are used only in the manufacturing process and not in the product. However, this does not apply to substances used in ways that are not directly involved in the manufacturing process, including analysis, measurement, and product development. (n) Presence of Substances Contained (cf. List A Substances Contained 0:No 1:Yes) Please input "1", when at least one 42 chemistry substance is contained. Please input "0", when 42 chemistry substances are not contained. Next, please click an "input" button. Please input substance information on a chemistry substance Survey screen. Note: 42 chemistry substances. When you click on the Input List A substances column, a list of the 42 substances is displayed. 4

(o) Note Please fill that in about the item which cannot be indicated for the reasons of the trade secrets of the information about composition parts etc. (3) Chemical substances survey(2) It is investigation about the situation of the chemistry substance of fundamental information investigation. Please input in principle all the substances that constitute the object item. (a) Amount Contained Enter the amount of chemical substance contained per the unit set in item (h) Unit, in the Basic Information Survey.(Integer maximum size ten Decimal point ten and below enter it in milligrams and round off after the ten digit.) (b) Application The application is the component of the part that contains the chemical substance subject to the survey. Enter the name of the application containing the substance for which you indicated the amount contained in (a). For the name of the application, enter the name used in specifications and diagrams, your usual term, or the general name for the application. When the item covered by the survey is a single electronic part or other part, the component is the item recorded on the diagram of the part concerned or the composition materials list. E.g. 1) Ceramic material, internal electrode, or external electrode in a layered ceramic capacitor E.g. 2) Lead wire, electrolytic solution, sealing material, or electrode foil in electrolytic capacitors E.g. 3) Rubber contact points, springs, plastic covers for switches When the item covered by the survey is a machinery product or assembly electronic part, the usage part is the item recorded on the diagram of the part (product) concerned, or the parts list. E.g. Layered ceramic capacitor, electrolytic capacitor, printed circuit board, or solder for assembly (c) Purpose of Use Enter the purpose of use for the substance contained, and other reasons for using that substance, in simple terms. E.g. 1) Stabilizer, plasticizer, coloring, flame retardant, rust preventative, solder ingredient, etc. E.g. 2) Main ingredient, heat stability improvement, electrical characteristic improvement, mechanical characteristic improvement, etc. E.g. 3) Impurity (when it is clear that it was not intentionally added), etc. 5

4.Notes for Survey of Chemical Substances Contained (1) Concept of "Contained" In principle, when the substance was intentionally added or is clearly present, the substance is considered to be contained regardless of the amount. When the substance was not intentionally added it is treated as an impurity. You are asked to record all possible impurities that can be measured. (2) Calculation of Amount Contained Indicate the amount contained using the amount under control, or the theoretical, calculated, designed amount, or actually measured. When there is variation in the amount contained in a manufacturing lot, indicate the maximum amount, in principle. Furthermore, the calculation of the amount contained in a part applies to the amount of chemical substance contained in the purchased components or materials that make up the part, as well as the amount contained in the manufacturing process. Retracing the purchased ones for the amount back to its supplier, determine this and enter the results. (3) Amount Contained in Metals and their Compounds For the amounts contained for metals and their alloys, enter the figure calculated for the amount of metal element. Note 1: For the metal conversion coefficients for compounds that are not included on the Example Substance List, calculate after checking the atomic weight using a chemical substance handbook. E.g. 1: To find the amount of antimony contained for a component containing 100 mg of antimony trioxide (Sb2O3), multiply by the conversion coefficient 0.835. Antimony amount = 100 mg 0.835 = 83.5 mg Æ 84 mg (4) Chemical Substances used in Processes Do not include any solvents or washing solutions used in the manufacturing process when they do not remain in the product due to their volatility. However, in cases where substances from the Survey Substance List are intentionally used in the manufacturing process, be careful as many of them are non-volatile and may remain in the product. (5) When the same substance falls under more than one survey substance group,amounts contained for each group. e.g. If the article contains lead chromate, indicate the amount of lead and hexavalent chromium contained for both "Lead and its compounds" and "Chromium VI compounds. " 6

(6) Ozone-layer Depleting Substances There are two types of ozone depleting substance surveys: the survey of substances used in the manufacturing process, and the survey of substances contained in the product. Respond to the survey of substances used in the manufacturing process by referring to section 3. (1) (k) Use of Ozone-depleting Substances in Manufacturing Process, and respond to the survey of substances contained in the product by referring to section 3. (2) (a) Amount Contained. (7) Bromide Flame retardants (excluding PBBs and PBDEs) For the amounts contained in bromide Flame retardants (excluding PBBs and PBDEs) provide either the CAS No. or ISO 1043-4 code.if entering the ISO 1043-4 code, the CAS No. need not be provided. (8) Radioactive Substances If a radioactive substance has been intentionally added, indicate the radiation level (in becquerels (bq)) rather than the amount. 7

Do not make entries or changes here. (2) About the parts number and parts name of Basic Information Survey When you fill in a part number or a part name, please be sure to fill both in. (3) About Composition information When there is entry in addition to the part number of basic information investigation, and part name, please be sure to fill in composition part and part weight. (4) About weight (content) The sum total value of investigation unit mass, part weight, and a chemistry substance content must be in agreement with the weight of a higher rank. part mass must be made into the sum total value of part weight 100% (coincidence). Part weight must be made into the sum total value of the contained substance 100% (coincidence). Note:Parts mass (10g) = Composition part weight sum total (10000mg) = chemistry substance content sum total (10000mg) Note:Parts mass is the "g" input. Part weight and a content are the "mg" inputs. (5) About cancellation of content chemistry substance investigation Even if it performs preservation (JGP output), when there is no input in fundamental information investigation and only the substance is written, contents chemistry substance information is not outputted. (6) New-line Please do not perform a new-line within a cell by all input items. 8

Attachment1: Common Green Procurement Survey Tool Ver.2.00 Operating Manual 1.Loading survey request data (1) Save the survey request data JGP file on your computer. (2) Double-click the Survey Tool V1.0.xls *1 (a) Excel *2 will start up. *1:Please note the actual file may be different in the case of a name change or a newer version *2:Excel is a registered trademark of Microsoft Corporation in the USA and other countries. (b) Click the LOAD JGP button. Select the survey request JGP file and click "Open. " The survey request will be loaded into the survey tool. click 10

2.Entering response details (1) Entering the surveyed company information (a) Enter your response date, company name, address, division name, contact name, telephone number, fax number, and email address. Since it sets up from an investigating agency about a reserve 4, please do not make a change. Note: If you are instructed to fill in columns 4 to 5 by the surveying company, please do so. Fill in this part [Entry Example] 11

(2) Enter the part information. When the items for investigation are two or more composition, like the following image, please shift a part number below and input it. The following part number is inputted into this place at the time of loading. In composition, please input following items for investigation after filling in all composition information. At the time of loading of a JGP file, since the part number is continuing, please shift a part number below and fill it in. (a) Please input the maker name and part number to the item for investigation. Select the unit and enter the total weight per unit : Select the unit from the pull down menu. : Enter the total weight in grams for the unit set. (e.g.) When the unit is "units" Æ the weight per unit of the survey item When the unit is kilograms Æ the weight per kilogram = 1000 g. [Entry Example] 12

(b) Please input composition part, manufacture maker, and part weight. Please input part weight by mg. Fill in surveyed company columns 1 and 2 based on the instructions of the surveying company. If not instructed to do so, do not enter anything or make any revisions. Please input Material norms or Material standars. Please input Material number or Material symbol. [Entry Example] ISO1043 JISG3445 EP STKM17C (c) Enter whether ozone depleting substances are used in the manufacturing process. : If they are used in the manufacturing process enter "1, " if they are not used enter "0. " Indicate whether such substances are used in the manufacturing process regardless of whether or not they are present in the part or product. However, this does not include uses outside of the direct manufacturing process such as analysis, measurement and product development. Please fill that in about the item which cannot be indicated for the reasons of the trade secrets of the information about composition parts etc. It is not necessary to enter without necessity. (Note) Please be sure to perform the input in details (substance). Click *If the compound weight (30 mg) is entered for antimony trioxide, the antimony amount contained will be calculated automatically from the composition ratio. [Entry Example] Fill in this part 15

When entering other antimony compounds, enter the Breakdown Substances, CAS No, amount contained, application, and purpose of use. Enter the antimony amount contained for the amount contained at that time. CAS No need only be provided if available. Click When the entries are complete, click the "OK" button. The calculated figures will be shown in the total sum. Click When you have finished entering all the substances contained click "OK, " and you will bereturned to the Basic Information Survey screen. 16

(h) When bromide flame retardants are contained (excluding PBBs and PBDEs) (For bromide flame retardants only: Respond with either the ISO 1043-4 code or CAS No.) Click "input" for B08 bromide flame retardants, If responding by substance name. (Example not provided as the screen is the same as for (g)) Click Enter the Breakdown Substances /CAS No. / amount contained / application / purpose of use / substance name In the case of other Brominated Flame Retardants, please input an illustration substance name (English). Fill in this part 17

(i) If a survey request was made for a substance other than those in the Survey Substance List Click "input" for C99 Other, If responding by substance name. (Example not provided as the screen is the same as for (g)) Click Enter the Breakdown Substances / CAS No. / amount contained / application / purpose of use / substance name in the C99001 Other1 column. Fill in this part 18

3 Saving the information entered in the survey response format (JGP file) (1) When all the entries are complete prepare to save the file. Note: It cannot save jgp file, unless each sum total value is in agreement with the weight of a higher Rank in order of investigation unit mass (g), part weight (mg), and a chemistry substance content (mg or Mbq). [Parts mass (10g) = Composition part weight sum total (10000mg) = chemistry substance content sum total (10000mg)] (a) Enter the response date. (b) Click "SAVE JGP" It is not a metal content. Click (2) Save in the JGP file format. (a) Check the "Save as type" is JGP (*.jgp) (b) Select the save in location, enter the file name, and click the "Save" button. Select the save in location Click the "Save" button Check that the save as file type is JGP (*.jgp). Enter the file name. Follow any instructions for the file name from the surveying company. 19

4.Preservation and closes of an survey tool. (1) When closing without saving an survey tool 1Select "File" then "Close" or "Exit" from the menu bar. (2) When it is in the middle of entry and a survey tool is saved 1Please display a basic information survey sheet. Note: Chemical substance survey (1) or in the case of (2), even a basic information survey sheet should return. Basic Information survey sheet 2Select "File" then "Save" from the menu bar. Note: If saved except a basic information investigation sheet, it will not operate correctly. 20
