1 NYSDOT TRANSPORTATION MAINTENANCE DIVISION REST AREA QUALITY ASSURANCE CONDITION ASSESSMENT Rating Guidelines SCORING INSTRUCTIONS The condition assessment is intended to be performed from a customer or user s orientation. It is n o t intended to b e a technical evaluation of rest area maintenance. For this reason the septic, mechanical, electrical o r heating systems a r e n o t included. The scorer is to assess the consequences or results of the maintenance effort. These guidelines aim at establishing a consistent set o f criteria f o r evaluating each rest area feature. Criteria a r e established f o r t h e A, C, a n d F condition. Since conditions may not exactly reflect one of these letter grades the scorer can use scores of B or D for intermediate conditions. Vehicle Count T h e scorer is to c o u n t t h e number o f passenger vehicles (cars, mini vans, ligh t t r u c k s ) a n d commercial vehicles (i.e. tractor trailers, busses, delivery vans, e t c. ) a n d n o t e t h o s e o n t h e s c o r e sheet. Overall Impression Beginning both t h e BUILDING section a n d t h e G R O U N D S A N D PARKING LOT section a space is provided f o r an Overall Impression. This is intended to be similar to the General Recommendation u s e d i n b r i d g e i n s p e c t i o n s. T h i s i s t o a l l o w f o r t h e s c o r e r t o a c c o u n t f o r a subjective reaction to the consequence of t h e entire maintenance effort. For example many small deficiencies that singularly may receive scores of C could lead to an overall impression of a reasonably well o r poorly maintained facility. Similarly, one significant deficiency such as excrement o r other filth o n t h e f l o o r, with all other elements being well maintained, would lead to an p o o r overall impression. This score should reflect on whether the facility leaves the overall impression of a good, mediocre, or poor maintenance. Comments For each feature an area is provided f o r comments. The intent is for the scorer express in very few w o r d s t h e rationale f o r t h e grade. For example under, Floors and Walls a score of B may include a comment like, Hairline wall crack. Floor swept, not mopped.
2 Winter and Non-Winter Scoring Note that in snow and ice conditions Snow and Ice Control should be evaluated in the parking lot section a n d ignored in t h e non-winter season. Lawn and Picnic Tables consequently are evaluated in the non-winter season, but not in the winter. Scoring T h e scorer is required only to p r o v i d e grades a n d comments d u r i n g t h e evaluation. Calculation of scores will b e d o n e in t h e office. Scores are calculated by multiplying the numeric equivalent of the letter g r a d e b y t h e percentage o n t h e s c o r e sheet. Letter grade equivalents are as follows: A = 4, B = 3, C = 2, D = 1, and F = 0. The best possible score is Scoring for a Closed or Inaccessible Building or Parking Lot In t h e extreme instance that a rest area sanitary facility is closed the entire BUILDING section should receive a s c o r e o f F. If the parking lot is inaccessible or closed the entire rest area should receive a s c o r e o f F. In the case of accessibility for the disabled any element that would make it impossible f o r a disabled p e r s o n f r o m accessing a stall and using the toilet will fail the entire ACCOMMODATIONS FOR THE DISABLED section. BUILDING E x t e r i o r R o o f A - New condition C- Showing age, but functional F- Failed Walls A - Good structural condition, attractively painted, and well insulated C- Minor structural deficiencies, moderate paint wear, walls moderately insulated F- Structural cracking, peeling paint, or no insulation D o o r s A - Fully functional, insulated, painted C- Minor f u n ctional deficiencies, moderate paint wear, some insulation/weather stripping problems F- Not functioning, damaged or missing Wall Maps A - Clearly readable, recent, and undamaged area map prominently posted C- Map > 5 years old, some deterioration F- No map - 2 -
3 Interior Sinks A - Basins clean, h o t & cold water with good pressure, porcelain and fixtures in new condition, washers recently replaced C- N o h o t water, g o o d water pressure, basins clean a n d in s o u n d condition, minor aging o f washers, fixtures functional, no more than one basin requires replacement F- Faucets d r i p p i n g, f i x t u r e s n o t f u n c t i o n a l, n o h o t w a t e r, p o o r p r e s s u r e, m o r e t h a n o n e b a s i n requires replacing Soap/Soap Dispenser A - Dispensers > half full of soap, fully functional, and clean C- One dispenser not functioning or without soap, dispensers < 1/4 full F- More than one dispenser empty or not functional Paper Towels / Hand Dryers A - Dispenser > half full, neatly stacked, hand dryers fully functional and well maintained C- Dispenser < 1/4 full, somewhat disorderly, h a n d dryers functional but requires some maintenance F- Dispenser empty or in serious disarray or hand dryers not functional Toilets A - All toilets clean, sanitized, d e o d o r i z e d, fully functional, each with more than half roll of toilet paper C- One toilet damaged, not functional, or unusable, or lacking toilet paper, others satisfactory F- More than one toilet unusable or damaged, no toilet paper, or unsanitary Partitions A - Partitions all in g o o d structural condition, free o f graffiti, d o o r s a n d locks o n stall s f u n c t i o n properly, coat hooks in place C- O n e partition o r stall d o o r missing o r with a hole o r other structural damage, some graffiti, somewhat unclean, coat hooks missing F- More than one partition or stall door damaged or missing Urinals A - All urinals clean, sanitized, deodorized, fully functional C- One urinal damaged, not functional, or unusable, others satisfactory F- More than one urinal unusable, damaged, or unsanitary Diaper Changing Tables A - Available in both men s and women s rooms, in good, clean, functional condition C- Available in women s room only, in good, clean functional condition F- Not available, unusable or not functional - 3 -
4 Trash A - Recepticals less than half full with no litter, liners as appropriate, deodorized C- Recepticals less than 3/4 full, one necessary liner missing, minor litter, little odor F- Recepticals overflowing, significant litter, needed liners missing, strong odor Environment / Odor A - Building is sanitized, clean, and deodorized, well maintained, and feels safe and secure C- Some untidiness, minor foul odors, incurs a sense of caution F- Feels a b a n d o n e d, exhibits a f o u l smell, appears unmaintained, seedy, a n d potentially dangerous, i.e., poorly lit areas in and out, or areas where someone could hide. Lighting A - Lighting and light covers in good condition, no opaque areas C- One bulb blown, one light cover missing, light covers need minor cleaning F- More than one bulb blown, light covers in poor condition or missing Windows A - Windows and casings are clean, in tact, functional and insulated, jambs are tight C- One window is broken or not functional, windows require minor cleaning or insulating F- M o r e than o n e w i n d o w is b r o k e n o r n o t functional, d i r t an d g r i m e h a s b u i l t u p o n w i n d o w s, outside air is significantly intruding through windows or casings Floors and Walls A - Floors a r e mopped clean a n d swept, surfaces a r e n o t cracked o r b r o k e n, walls are attractively painted and clean C- Floors swept b u t n o t mopped, minor surface cracking, p a i n t has dulled a n d s h o w i n g some age F- Floors are dirty, surfaces show significant cracks or breaks, paint is peeling or chipping Mirrors A - Mirrors are all present, clean, and fully reflective C- One mirror is missing, broken or needs significant cleaning, some lost reflectivity F- More than one mirror broken, missing or unusable Temperature A - Interior spaces are comfortable, temperature between 68 and 72 degrees C- Interior spaces a r e slightly warm o r cool, departing f r o m 6 8 a n d 7 2 degrees b y n o m o r e than 8 degrees F- Interior temperature below 60 degrees or above 80 degrees Vending Machines A - Vending machines clean, stocked, and fully functional C- One selection sold out, machine needs minor cleaning F- M o r e than o n e selection sold out, machine dirty or shows graffiti, money handler n o t w o r k i n g properly, refrigeration not working properly, machine not working - 4 -
5 Pay Phones A - Phones are fully functional, clean, free of gum and graffiti C- Minor amounts of dirt, gum or graffiti not hindering the function of the phone F- Phone damaged, coin or card insert blocked or not functional, serious dirt or grime Water Fountains A - Fountains clean, p r o v i d e g o o d water pressure, water clear, clean, cool, potable a n d has n o f o u l taste, preventive maintenance including filters has been done C- Fountains clean, water p r e s s u r e adequate, water clear, clean, cool, potable, a n d w i t h n o f o u l taste, some preventive maintenance indicated F- Fountain missing, inoperable, water turbid o r f o u l tasting, n o indications o f preventive maintenance G R O U N D S Lawn A - Turf is strong, grass less than 4" in height, few weeds, no bare spots C- T u r f in g o o d condition, grass less than 8 " in height, some weeds, o c c a s i o n a l b a r e p a t c h e s o r paths F- Turf is unhealthy, grass > 8", infested with weeds or pests, significant worn areas Picnic Tables A - Tables painted, in s o u n d structural condition, surface a n d b e n c h es a r e s m o o t h, f r e e o f f o u l smells, or food debris, graffiti, and pests C- Minor p a i n t wear, in s o u n d structural condition, surface has some minor irregularities ( n o t h i n g sharp), some graffiti, free of foul smells or pests F- Significant paint wear, structural problems, sharp protrusions, foul odor, or pest infestation Trash and Litter A - Recepticals less than half full with no litter, liners as appropriate, deodorized C- Recepticals less than 3/4 full, one necessary liner missing, minor litter, little odor F- Recepticals overflowing, significant litter, needed liners missing, strong odor Trees, Shrubs & Plantings A - All trees, s h r u b s a n d decorative plantings a r e healthy a n d properly trimmed, dead o r u n s a f e trees are removed C- A few trees, s h r u b s o r plantings s h o w signs o f declining health, o r a lack o f p r u n i n g o r trimming, not receiving proper water or fertilizer, most are healthy F- Trees or plantings dead or in severe decline, or removed plantings not replaced - 5 -
6 Fencing and Walls. A - Fences, retaining o r barrier walls a r e in s o u n d struc t u r a l c o n d i t i o n, a n d a r e i n p l a c e w h e r e necessary, no unsafe conditions exist C- Fences or walls show some structural deterioration or lean slightly F- Fen ces or walls are breached, or not in place where necessary, or show severe structural deterioration or damage Pet Walking Area A - Where space is available, a pet walking area is in place, signed, mowed and kept reasonably clean C- The area shows some build up of excrement, or a minor lack of care F- Pet walking area is needed and does not exist, or existing area shows a significant lack of maintenance PARKING LOT Pavement Condition A - Pavement s h o w s n o o r i n f r e q u e n t distress a n d n o to very slight severity ( ), with n o standing water, ice or debris caused by improper paving or settling C- Pavement s h o w s slight to moderate distress that is occasional to f r e q u e n t ( 5-7 ), minor standing water, ice or debris F- Pavement s h o w s severe to very severe distress that is f r e q u e n t to very f r e q u e n t ( 1-4 ), significant standing water, ice build-up or debris Sidewalk Condition A - Sidewalk shows no or infrequent distress and no to very slight severity C- Sidewalk shows slight to moderate distress that is occasional to frequent F- Sidewalk shows severe to very severe distress that is frequent to very frequent Drainage A - Ditches a r e clean, properly g r a d e d, s i d e s l o p e s a r e s t a b l e a n d m o w e d w h e r e p o s s i b l e w i t h n o standing water, closed drainage systems a r e clean, in g o o d structural condition, functioning as designed, grates are free of debris and in a good and safe condition C- Ditches s h o w some sediment build-up, have been over dug, need minor regrading, or need mowing, closed drainage system s h o w i n g a small amount o f material b u i l d - u p, minor structural deficiencies, some erosion around end sections or head walls, some splash protection needed, grates or manhole covers showing some age F- D itches infiltrated with sediment, ponding, sideslopes cut to > 1:2, cattails or other wetland species present, closed drainage severely restricted with material, significant structural deterioration or failures, end section or headwalls being eroded and undermined - 6 -
7 Signing A - Signs a r e located a n d mounted properly, easily readable and understandable, fully reflective, possess adequate letter height C- Signs showing some wear, loss of reflectivity, minor damage F- Signs hard to read, improperly placed, missing, obscured by vegetation Striping A - Edge stripes a n d p a r k i n g spaces properly marked, stripes a r e clearly visible a n d reflective, old markings removed when necessary C- Stripes showing some wear and minor loss of reflectivity, a few old markings present F- Stripes significantly faded, significant loss of reflectivity, missing, improperly placed Commercial Vehicle Parking A- Ample signed parking for large commercial vehicles, separated from our parking area C- Some parking available, but inadequate for volume F- No separate commercial parking area, commercial vehicles disrupting vehicle flow in lot Lighting A - All b u l b s in lighting standards fully functional, covers a r e clean a n d translucent, all lights come o n a t d u s k C- One bulb missing or failed, covers require minor cleaning, some graffiti on standards F- M o r e than o n e bulb missing o r failed, covers failed o r significantly under maintained, excessive graffiti on standards Snow and Ice Control A - Pavement a n d sidewalks free o f s n o w o r ice b u i l d - u p, r o o f o f building has minor accumulation o f s n o w o r i c e C- Some s n o w b u i l d - u p on pavements or sidewalks, moderate snow on roof of building, few ice patches F- Significant s n o w and ice build-up on pavements or sidewalks, rutting, hazardous amount of s n o w b u i l d - u p o n t h e r o o f o f b u i l d i n g ACCOMMODATIONS FOR DISABLED PERSONS Parking Spaces A - Clearly signed a n d striped 9 6 in c h p a r k i n g s p a c e s w i t h 9 6 i n c h p a s s e n g e r l o a d i n g a i s l e s, positioned close to t h e building o n n o more than a 2% grade, in adequate number (defined b y the total number of spaces), and free of snow, water or debris C- Sign or striping fading, minor snow, water or debris in space or access aisle F- P a r k i n g area o u t o f compliance with t h e Americans with Disability Act Accessibility Guidelines f o r Buildings a n d facilities ( A D A A G ) a n d / o r m o r e restrictive N Y S requirements outlined in t h e Dept s Access for People with Disabilities Reference Guide
