1 COLLABORATION AGREEMENT (hereinafter referred to as the Agreement ) between THE EUROPEAN ORGANIZATION FOR NUCLEAR RESEARCH (hereinafter referred to as CERN ), an Intergovernmental Organization with its seat at Geneva, Switzerland, Host laboratory of the NA61/SHINE experiment, represented by its Director for Research and Scientific Computing, Sergio Bertolucci and THE INSTITUTION NAME (hereinafter referred to as IN ), with its seat in institution seat, represented by the Director of IN, NN. CERN and IN are hereinafter collectively referred to as the Parties or individually as Party. TAKING INTO ACCOUNT THE FOLLOWING CONSIDERATIONS: - The Letter of Membership in NA61/SHINE dated on date sent by the NA61/SHINE spokesperson to IN on behalf of the Collaboration Board of International Scientific Collaboration NA61/SHINE; - IN is a participating institution (hereinafter referred to as Participating Institution ) in NA61/SHINE International Scientific Collaboration hosted by CERN (hereinafter referred to as NA61/SHINE ), under the terms of the Memorandum of Understanding for Collaboration in the NA61/SHINE experiment for study of Hadron Production in Hadron-Nucleus and Nucleus-Nucleus Collisions at the CERN SPS dated 24 October 2008 between CERN and the Participating Institutions (hereinafter referred to as the MoU, );
2 THE PARTIES HAVE AGREED AS FOLLOWS: Article 1 Scope of the Agreement IN and CERN shall carry out the scientific work plan of NA61/SHINE as set forth in Annex II, and this in accordance with the terms and conditions of this Agreement and the MoU. In case of discrepancy between the terms and conditions of this Agreement and the MoU, the latter shall prevail. Article 2 Duration of the Agreement This Agreement shall start on date and continue until the MoU remains in force, according to the Article 12 of the MoU. The expected duration is about 10 years. Article 3 Financial Contribution by IN and NA61/SHINE 1. The cost of the annual financial contribution from each Participating Institution mentioned in Annex I, including IN, to the NA61/SHINE common fund in year, is amounting to CHF/Participating Institution (the Amount ). The payment of the Amount of the Participating Institutions aims to promote the running of NA61/SHINE and, more in particular, reimbursing the coverage of the NA61/SHINE expenses (expenditures) on preparations and running the NA61/SHINE installation during the in-beam data taking and tests in line with the plan specified in Annex II. The Amount may be changed by a decision of the Collaboration Board to adapt to future needs of the experimental programme. 2. IN shall pay the Amount in accordance with the payment details in Annex III. Exception to this rule may be granted in special cases by the Collaboration Board. Failure or delay in payment of the Amount may give rise to a decision by the Collaboration Board to terminate the Agreement, according to the provisions of the MoU, Article 10. Article 4 Amendments, Variations or Additions Any modifications to the terms and conditions of this Agreement can be made only by means of a written agreement signed by authorized representatives of the Parties. Article 5 Suspension and Termination of the Agreement 1. Suspension and termination procedures of this Agreement can be made only by means of a written agreement signed by authorized representatives of the Parties. 2. Notwithstanding the foregoing, the relevant provisions of the Article 3, par. 2 of this Agreement are binding. Article 6 Names and logos Neither Party shall use the names and logos of the other Party without its prior written consent, if it is not directly related to the performance of obligations under the Agreement

5 ANNEX II Work Plan Summary NA61/SHINE studies hadron production in hadron-proton, hadron-nucleus and nucleus- nucleus collisions at the CERN Super Proton Synchrotron. The NA61/SHINE physics goals include: (i) (ii) Measurements for the physics of strong interactions, in particular, the study of the onset of deconfinement and search for the critical point of strongly interacting matter which are pursued by investigating p+p, p+pb and nucleus-nucleus collisions. Measurements needed to improve calculations of the initial neutrino beam flux in the longbaseline neutrino oscillation experiments as well as for more reliable simulations of cosmic-ray air showers. The work plan is based on the plan of the NA61/SHINE Collaboration presented in the NA61/SHINE Proposal SPSC-P-330; CERN-SPSC ( ) and the Memorandum of Understanding ( ). The latest update of the work plan is given in the 2014 NA61/SHINE Status Report CERN-SPSC ( ) The tasks and responsibilities of the IN NA61/SHINE Team within NA61/SHINE are:
