Poisoning 62

Poison May Enter The Body in Four Ways Swallowing or Ingesting: Medication, Household Cleaners and Chemicals.. Inhaling: Noxious dusts, Gases or mists.. Penetrating the Skin: Animal, Snake, Insect Bites.. Absorbing through Skin: Poisonous Liquids

Signs & Symptoms May Include Burns or redness around the mouth and lips Breath that smells like chemicals Vomiting Difficulty in breathing Drowsiness Confusion or Altered mental status Uncontrollable restless or agitated Known to have taken Having seizures 64

Pre Hospital Treatment Swallowing or Ingesting: Remove anything remaining in the persons mouth If the suspected poison is a household cleaner or other chemical, read the container s label and follow instructions for accidental poisoning. 65

Pre Hospital Treatment Inhaling: Get the person in fresh air as soon as possible Have somebody gather pill bottles, packages or containers with labels, and other information about the poison, send along with ambulance team. 66

Pre Hospital Treatment Penetrating the Skin : Remove any contaminated clothing using gloves Rinse the skin for 15 to 20 minutes in a shower or with hose Poison in the Eye: Gently flush the eye with cool or lukewarm water for 20 minutes or until help arrives 67

Burn Emergency An injury caused by exposure to heat or flame 68

Management of Patient With Burn 69

Causes of Burn Burns are caused by a transfer of energy from a heat source to the body. Heat may be transferred through conduction or electromagnetic radiation. Burns are categorized as thermal (which includes electrical burns), radiation, or chemical. 70

Causes of Burn Disruption of the skin can lead to increased fluid loss, infection, hypothermia, scarring, and changes in function, appearance, and body image. 71

Classification of Burns and Pre Hospital Treatment 72

Superficial Burn Injuries (first degree burn): In a superficial burn, The epidermis is destroyed or injured. A portion of the dermis may be injured. The damaged skin may be painful and appear red and dry, as in sunburn, or it may blister (very minimal). 73

Superficial Burn Injuries (first degree burn): Pre Hospital Treatment: Cool the burn to help soothe the pain. Hold the burned area under cool (not cold) running water for 10 to 15 minutes or until the pain eases. Or apply a clean towel dampened with cool tap water. Remove rings or other tight items from the burned area. Try to do this quickly and gently, before the area swells. Consider a tetanus shot. Make sure that your tetanus booster is up to date. Doctors recommend people get a tetanus shot at least every 10 years. 74

Partial-Thickness Injuries. (second degree burn): A partial-thickness burn involves destruction of the epidermis and upper layers of the dermis and injury to deeper portions of the dermis. Large blisters over an extensive area The wound is painful, appears red, and exudes fluid. Capillary refill follows tissue blanching. Partial-thickness burns take longer to heal and are more likely to result in hypertrophic scars. Sensitive to cold air Healing in 2-3 weeks Grafts may be needed 75

Partial-Thickness Injuries. (second degree burn): Pre Hospital Treatment: Protect the burned person from further harm. If you can do so safely, make sure the person you're helping is not in contact with smoldering materials or exposed to smoke or heat. But don't remove burned clothing stuck to the skin Check for signs of circulation. Look for breathing, coughing or movement. Begin CPR if needed. Remove jewelry, belts and other restrictive items, especially from around burned areas and the neck. Burned areas swell rapidly. 76

Partial-Thickness Injuries. (second degree burn): Pre Hospital Treatment: Elevate the burned area. Raise the wound above heart level, if possible. Cover the area of the burn. Use a cool, moist, bandage or a clean cloth. 77

Full-thickness injuries. (third degree burn): A full-thickness burn involves total destruction of epidermis and dermis and, in some cases, underlying tissue as well. Wound color ranges widely from white to red, brown, or black. The burned area is painless because nerve fibers are destroyed. The wound appears leathery; hair follicles and sweat glands are destroyed. Tissue open with fat exposed Takes weeks to months to heal 78

Full-thickness injuries. (third degree burn): Pre Hospital Treatment: Don't immerse large severe burns in cold water. Doing so could cause a serious loss of body heat (hypothermia) or a drop in blood pressure and decreased blood flow (shock). Hospitalize the Patient: Immediately 79

Remember Don t Remove burned clothing that is stuck to the body. Don t Use very cold water or ice on a burn. It can cause more skin damage. Don t apply ointments, jellies, sprays, creams or butter. (Even In the case of a severe burn) Don t Break open small blisters with clear fluid inside because this could let germs into the wound. Don t Assume the burn injury is minor. Even a small burn can be serious depending on its location and the age of the victim. 80
