Page 2 of 7 Pre-existing Violations: Rule: 62-701.300(3) Question Number: 5.5 Explanation: On July 29, 2015 smoke was observed from subterranean fire on the west side of the disposal area. Seven smoking areas were flagged with orange wire flags where smoke was rising through the dirt/clay cover. On August 6, 2015 two areas were observed smoking on the southwest corner of the facility's disposal area. Corrective Action: Apply additional dirt/clay cover or whatever steps are necessary to immediately extinguish the fire. COMMENTS: 10/01/2015 On October 1, 2015, Brad Hartshorn and I visited the Waste Pro Freeport C&D Disposal Facility to check the status of the fire. Per Dara at the entrance building, they had approximately 7 inches of rain in the last two days. Most of the dirt stayed on the slopes during the rain. No smoke or flames were observed today, however we could smell smoke as we walked around on the haul road. We did not walk on the slopes for safety reasons. We did find one hot spot near the unauthorized waste dumpsters on the flat area of the the top west slope. We suggested to Kevin to move the dumpsters and apply a thick layer of dirt cover material in that area. He stated that he would do that. Approximately ½ of the west slope including the southwest corner has been covered with on-site sand and import clay material. There are still many subsidence s on the west and north slopes. I called Mark Cannady and we discussed the inspection. He stated that he will have another man out there tomorrow and they will continue to place dirt on the slopes. I asked him if they were going to get additional clay dirt from offsite and he stated No. Photo Attachments: -Photo 9 shows the north slope with some sand and some clay cover. -Photo 10 shows the west slope with sand and clay cover. -Photo 11 shows a small area of erosion on the west slope (near SW corner) from recent heavy rains. Note thickness of cover dirt in this area is roughly 5 inches. -Photo 12 shows the slope at the SW corner with cover dirt and subsidence areas near toe of slope. -Photo 13 shows a hotspot area near the unauthorized waste dumpster near west slope. -Photo 14 shows overview of unauthorized waste dumpsters and proximity to west slope in background of photo. The area next to the dumpsters is where a hot spot was located during this inspection. -Photo 15 shows areas of subsidence near the center of the west slope, close to toe of slope. 10/12/2015 On October 8, 2015 a follow-up inspection was conducted at the Waste Pro Freeport C&D Disposal Facility to check the status of the fire. I could smell a little bit of a smoke odor near the north slope and the southwest corner, but saw no smoking areas or flames. I walked on both the upper access road and along the toe of the slope, staying off of the primary slope. Wind was out of the northeast at about 10+ mph. Hydrogen sulfide odor was strong near the top of the west slope by the large wire spools staged there. Many truckloads of waste came into the facility while I was there. Kerrell Johnson (Tater) stated that he thought 80 truckloads had come through the gate by 2pm,.
Page 3 of 7 Several subsided areas still exist. Cover dirt has been placed on the upper tier of the west slope, where the unauthorized waste dumpsters were staged. Photos taken this week look virtually the same as last week with the exception of dirt placed in the hot spot area after dumpsters were moved (1st photo) and they had a few truckloads of dirt staged at the top of the west slope for cover placement (2nd photo). I called Mark Cannady after I left the facility and he stated that they will continue to place dirt on the slopes. An additional man will be on site tomorrow to push dirt and cover various areas. He assured me that they are in this for the long haul and will do what it takes to put the fire out. Photo Attachments: -Photo 16 shows cover dirt staged on the top of the west slope. -Photo 17 shows where cover dirt was placed on the hot spot area notice during last inspection. -Photo 18 shows a portion of the north slope. Photo was taken from the toe of the north slope. 09/28/2015 On September 24, 2015 a follow-up inspection was conducted at the Waste Pro Freeport C&D Debris Disposal Facility to primarily check the status of the recent fire in the disposal area. Shawn Hamilton, Andy Joslyn, Mark Cannady and I walked the facility. Several of the slopes (west, north and southwest corner) have recently been covered with dirt of various thickness. However, 5 separate areas of smoke and several other hot spot areas were observed during the inspection. One area close to the center of the west slope had a cavern-like opening and flaming, burning wood could be seen down inside the hole. Some of the hottest areas are located on the southwest corner of the facility and on the north slope near the haul road. Mark instructed workers to place additional dirt in these areas. We discussed the need to place additional dirt and possibly put additional workers on site that strictly deal with the fire and covering the smoking areas. Orange wire flags were placed at all smoking areas. Mark called me later in the day and informed me that there will be an additional man on site 9/25, 9/26 and 9/27 if needed, to place cover dirt on the disposal areas that have been smoking/burning. He also stated that Gulf Coast Utility Contractors will be hauling in a clay type dirt to be used as cover. Photo Attachments: -Photo 1 shows an open hole with burning debris inside. This area was located near the middle of the west slope, toward the toe of slope. -Photo 2 shows workers placing dirt on the area noted in Photo 1. -Photo 3 shows Mark Cannady pulling out a piece of plywood from an area on the west slope that was smoking. -Photo 4 shows a smoking area located on the north slope. -Photos 5 and 6 show dirt cover that was recently placed on upper tier waste of the north slope. -Photo 7 shows a smoking area located on the north slope near then upper tier of waste. -Photo 8 shows workers (dozer on left side of photo) placing dirt on the area mentioned in Photo 7.
