3 Common drug testing discrepancies BC Bottle leaked in shipment, quantity not sufficient to test. SC Specimen quantity not sufficient to test. SE Specimen volume < 30 ml. Situation Lid not tighten properly. Service member provided less than 30 ml (BC) Ensure service member tighten cap properly. It could create possible contamination to other samples in the box. Decreased quantity (<30ml). (SC and SE) Keep the service member in the area until able to provide >30ml. Minimum volume of specimen should be annotated in Block 13 of DD Form 2624.

6 Common drug testing discrepancies BD BF Broken seal Bottle, two seals, no explanation Situation Service member broke the seal and placed a new one on top of the other seal. To correct Codes BD and BF, an annotation on DD Form 2624, block 13 must be made. It should include batch and specimen number and an explanation. Block 13 is indicated by a green arrow on the next page.

8 Common drug testing discrepancies FA Form-UIC or base/area code discrepant/differs from bottle Situation When incorrect UIC/Base Area Code is used. To correct Code FA, the SACO must correct information within the Pool Maintenance of the Drug Testing Software, DTP Lite or ensure the NDSL has the correct RUC for their unit. Examples in the next two pages.

To correct Code FN, ensure dates (12a) are indicated every time the box of samples are transferred to a person or location. Released by (b) and released to (c) are properly filled out printed and signed. And reason/purpose of transfer (d). The last entry on (d) should be a stamp from USPS. Example on the next page.

13 Common drug testing discrepancies FT Form SSN discrepant LN Label SSN does not match the form. LX Label SSN discrepant. Situation When correcting SSN, other forms were not corrected at the same time. To correct Codes FT, LN and LX, when forensically correcting SSN, ensure the SSN on the label, register and DD Form 2624 are corrected at the same time. Example on the next page.

15 Common drug testing discrepancies GG GR Form listed specimen, no bottle received. Form marked void for received specimen Situation It happens when the wrong SSN is voided. To correct Codes GG and GR, ensure you are forensically correcting the SSN of service member who did not provide sample. Take your time.. Example on the next page.

17 DRUG TESTING MAILING PROCEDURES

18 Mailing Line your box with a plastic bag. Under the bottles, you should have a absorbent pad. Arrange your samples according to samples numbers using the arrow (a) as your starting point. Arrow (b), the batch number should be written on outside of the box. (a) (b)

19 MAILING Seal or tie a knot to close the plastic bag. Place the original and 1 copy of the DD From 2624 on top of the specimen as shown. Original and 1 copy

20 MAILING Once you closed the box and taped at least 1 ½ revolution around, INITIAL and DATE the box. You will receive a PD discrepancy without your initial and date.

21 MAILING Place your box in the white mailing bag. Mailing address and return address as shown. DO NOT forget the Biohazard label. You may put urine sample below the word biohazard. You are ready to mail your sample. If feasible, mail your sample the same day of collection or you may store the sample in a secured space NO MORE THAN 72 HOURS.

22 MAILING The USPS (Military section) will stamp your DD Form 2624 as shown. That ends your chain of custody responsibility.
