Overview: What is the QM Green Light Report? The Green Light Report (GLR) is an online summary of Quality Measures outcomes for a group. Each report features drill down capability to facility and resident level details. Benefits of using the GLR include: Easy access to MDS data for multiple facilities Aggregated display of facility performance for Quality Measures (QMs) MDS File Submission (Submission Compliance) 4

Overview: Create Reports To access Green Light Report (GLR), use the dropdown menus to: Choose a Category Specific categories will depend on your active products. Select a Measure Select a performance measure. The Measure list updates based on the selected Category. Select a Time Period The reporting period is a rolling quarter and includes all assessments with a qualifying reason for assessment (RFA) according to the measure definition. 5

Overview: Percentile Range The Green Light Report (GLR) scores facility performance Percentile Range The reports display color coded categories based on the facility percentile: Red = needs improvement Yellow = average performance Green = top performers The Percentile Range can be customized using the Red and Green dropdown menus. The system remembers the most recent changes made to the Percentile Range. 6

Overview: Percentile Range (cont.) The Number of Facilities columns display the number of facilities by division or region within each score range. Note: Specific scoring details are presented later in this training. 7

Access the Green Light Report If you have management permissions you will be able to view the Green Light Report from the Location and Products pages. Depending on your role and permissions, you may also be able to drill down to Resident level details. The following slides will guide you through the steps for accessing the QM GLR Step One: Login to PointRight Step Two: Select the Report you want to see Step Three: Review the Reports 8

Step One: Login to PointRight 1. Go to: www.pointright.com 2. Click Client Login 9

Step 2: Access the Green Light Report 1. Click the Location tab to access your Corporate facility list 3. Select Products and click to view the Products page 12

Step 2: Access the Green Light Report (cont.) Green Light Reports (GLRs) are available for each of the products that are activated for your Corporation. Select the GLR you want to see from the Products list. On this screen we have selected the QM Green Light Report. Click to view the QM Green Light Report 13

Quality Measures (QM) Green Light Report: Overview The QM Green Light Report provides Percentile ranking and rates for each CMS Quality Measure for the selected time period. The four Surveyor Measures are included Use the QM Green Light Report to: Enhance your understanding of CMS Quality Measures Display the Short Stay and Long Stay Measures allowing for facility and group level summary information Take a proactive approach to quality improvement incorporating the QM Data 16

Quality Measures (QM) Green Light Report: Performance Performance can be compared to peers and is displayed as a Percentile Percentile score is based on the selected group or facility s score relative to the PointRight National Benchmark Comparison ranges can be customized using the drop down menus Default Percentile range is shown below: Percents (Rates) are based on the number of residents triggering the selected measure (Numerator) relative to the total number of eligible residents (Denominator) 17

QM Green Light Report: Components Components of the QM Green Light Report include: Selectable Quality Measures and Time Period (report covers rolling quarters) Customizable Performance comparison ranges Percent % (Rate) display option Drilldown reporting capabilities Export to Excel spreadsheet 18

QM Green Light Report: Categories The QM Category is the default for the QM Green Light Report. You can also select File Submission to view the percent of MDS submissions for the selected month. 19

20 QM Green Light Report: Measures The QM Measure defaults to All Measures. To view results for a specific QM, make your selection from the list of Measures in the drop down menu.

QM Green Light Report: Time Period The Time Period defaults to the most recently calculated period. To view results for another time period, select it from the Period dropdown menu. The screen updates automatically. Report time period is a rolling quarter 21

QM Green Light Report: Percentile Range The Percentile Range ranks your facilities against other facilities in the PointRight National Group. The Percentile for groups uses an average of the facility percentiles to account for varying sizes of facilities within a group. The Number of Facilities columns display the number of facilities within each Percentile Range for the QM Measure 22

QM Green Light Report: Percentile Range (cont.) The Percentile Range ranks facilities and groups in a Score Range from 0 100. A higher ranking indicates poorer performance relative to the comparison group, and most likely needs review A lower ranking indicates better performance relative to the comparison group. 23

QM Green Light Report: Percentile Range (cont.) You can customize the Performance Score within the red and green ranges using the dropdown menus. The yellow mid range updates automatically. The system remembers the last range you selected. 24

QM Green Light Report: Percent The Percent is the percent of residents who triggered for the selected measure compared to the total number of residents considered for the measure for the selected time period. Percent ranges correspond to the active Percentile Tip: To ensure accurate reports, use the File Submission category to check submission compliance for the selected time period. ranges for the selected Measure. QM Green Light Report: Percent (cont.) If All Measures is the selected Measure: The Percentile value is the group average percentile for the selected facilities/groups The Percent value is the group average for each measure for the selected facilities/groups 26

QM Green Light Report: Export to Excel Click the Export to Excel button to produce a spreadsheet of the QM Green Light Report From the top level of the report (All Measures/All Facilities), the spreadsheet will include three tabs: Percentile: The number of facilities in each performance level according to percentile Percent: The number of facilities in each performance level according to percent Both: All facilities percentile and percent for all measures 27

QM Green Light Report: Measure Drilldown If All Measures is selected, click on a Measure to view the facility rank for the selected measure 29

QM Green Light Report: Facility Drilldown Click on a facility name to view the facility s QM Trend report for the selected measure Tip: Once you drill down into a facility s QM Trend you can navigate to that facility s reports and drill down to resident level details. Print the QM Green Light Report Roll the mouse icon over the Print tab Select Print Report from this Page A printable version of the report page will be generated in a new window. Click the Print button in the printable window to print. 32

QM Green Light Report: Tips for Success Use the QM Green Light Report to: Identify facilities with high QM percentiles or pattern of decline Identify areas where additional chart review is needed Determine high risk clinical areas Evaluate staffing and resource needs Identify potential educational needs 33
