HAZARD COMMUNICATION PROGRAM 1.0 PURPOSE The Indiana University (IU) is to: 1.1 Inform IU employees of the hazards associated with the chemicals in their workplace 1.2 Provide information to employees on appropriate protective measures for the chemical hazards in the workplace 1.3 Meet employees Right-to-Know about the chemical hazards in their workplace 1.4 Comply with the requirements of the Indiana Occupational Safety and Health Act (IOSHA) Hazard Communication Standard and other state and federal regulations as applicable 2.0 REGULATORY REFERENCE 29 CFR 1910.1200, IOSHA s Hazard Communication Standard 3.0 SCOPE This document is intended as a master document that addresses regulatory requirements. Details specific to implementing the program for a specific department or job can be added by the user in the appendices of this program. This program is applicable to all employees of Indiana University, including contractors and subcontractors. For laboratory employees, 29 CFR1910.1450, the IOSHA Laboratory Standard takes precedence. Indiana University is required to develop, implement, and maintain at each workplace a written. This document is intended to meet that requirement. This program is applicable to all hazardous chemicals used by employees for work-related activities. Hazardous is applied to chemicals which may, when used, pose a physical hazard or health hazard to employees. Certain chemicals are specifically exempted, including pesticides, fungicides, and rodenticides (covered by other regulations) food, food additives, drugs, cosmetics and medical or veterinary products. A more complete list can be found in the IOSHA standard. 4.0 ELEMENTS OF PROGRAM 4.1 Chemical List/Inventory: There must be a list of hazardous chemicals known to be present in the workplace. At Indiana University, this list is developed and maintained separately for each individual work area by the manager or supervisor for that area and is available within that work area. In addition, the Indiana University Office of Environmental, Health, and Safety Management (EHS) maintain a list of all hazardous chemicals on the Indiana University Bloomington (IUB) campus. Each regional campus maintains the list for that campus. The chemical list is also used as the reference for required Material Safety Data Sheets (MSDS). A MSDS must be available for each chemical on the list. Page 1

4.2 Informing Employees of Hazards: Each worker will be informed about the hazardous chemicals used routinely and non-routinely in the workplace through: 4.2.1 Initial training at the time of hire or assignment to a new job 4.2.2 Training whenever hazardous chemicals in the workplace change or use of the hazardous chemical changes 4.2.3 Access to the Material Safety Data Sheet for each hazardous chemical in the workplace 4.2.4 Labels on the hazardous chemical container. 4.3 MSDS or Hazard Evaluations: Indiana University is not required to evaluate the hazards of each chemical in the workplace, but may rely on information provided by the manufacturer or distributor (provided as the Material Safety Data Sheet for the chemical). IOSHA requires that the MSDS include the following information: 4.3.1 The chemical name and common name 4.3.2 Physical and chemical characteristics 4.3.3 Physical hazards 4.3.4 Health hazards, signs, symptoms 4.3.5 Primary routes of entry 4.3.6 Specific exposure limits 4.3.7 Data on carcinogenicity, if applicable 4.3.8 Safe handling procedures 4.3.9 Control measures 4.3.10 Emergency and first aid procedures 4.3.11 Date of preparation of the MSDS 4.3.12 Contact information for the manufacturer or distributor 4.4 Access to MSDS: On-site access to the material safety data sheet for each hazardous chemical on this list must be readily available to all workers in the individual work area. These MSDSs may be available as a written, hard copy kept at the work site, or accessed electronically, so long as each worker knows how to obtain each MSDS and is able to do so at any time during the work period. 4.5 Label: Each hazardous chemical at Indiana University must bear a legible label with at least the following information, either as text, pictures, or symbols: 4.5.1 The identity or the name of the chemical 4.5.2 Appropriate hazard warnings 4.5.3 Name and address of the chemical manufacturer or distributor The label may also include information such as Chemical Hazard Ratings, (i.e., fire, reactivity, health, other). (See Appendix B1 for detailed labels) Removing or defacing labels on hazardous chemicals is strictly prohibited. If a label does become damaged, is missing, or is unreadable, the container must be relabeled with the above information prior to use. Page 2

If a hazardous chemical is transferred to another container (e.g., a smaller container), that container must also be appropriately labeled, unless the container: a) remains at all times in the possession of the person transferring the chemical, and b) is used only during that work shift. 4.6 Written : This document constitutes Indiana University s written. The document is available from the Office of Environmental, Health, and Safety Management and via the office s website: www.ehs.indiana.edu 5.0 ADMINISTRATION/COMPLIANCE/RESPONSIBILITIES Indiana University is responsible for identifying and listing all hazardous chemicals in its workplaces, obtaining MSDSs and labels for each hazardous chemical if not provided by the distributor, developing and implementing a written hazard communication program, and communicating hazard information to employees through labels, MSDSs, and formal training. IOSHA identifies four elements that are critical to the development of a successful safety and health management program: 5.0.1 Management commitment and employee involvement 5.0.2 Worksite analysis 5.0.3 Hazard prevention and control 5.0.4 Safety and health training 5.1 The Indiana University is administered by the IU Office of Environmental, Health, and Safety Management. EHS is responsible for developing and updating the program as appropriate. EHS makes the written program available, both in written format and on the EHS website. EHS may provide employee training to meet the requirements of the program when requested by departments, managers or supervisors. EHS maintains training records for training provided by them. Additionally, EHS can assist supervisors in evaluating hazardous chemicals and recommending appropriate engineering controls, administrative controls, and personal protective equipment. 5.2 Department heads, managers, and supervisors are responsible for implementation of all elements to their employees. These elements are found in Section 4.0 and include making MSDSs for all chemicals in the workplace readily available to employees and providing appropriate personal protective equipment. 5.3 Employees, once trained on the hazardous chemicals in the workplace, are responsible for recognition of the associated hazards and working in a safe and healthful manner, familiarizing themselves with the label and MSDS information for each hazardous chemical, following standard procedures, and using appropriate personal protective equipment. 6.0 REQUIRED TRAINING AND RECORDKEEPING 6.1 Employee Information and Training: Indiana University will provide employees with effective information and training on hazardous chemicals in their work area: 6.1.1 At the time of their initial hire or assignment to a job, provided either by the IU Office of Environmental, Health, and Safety Management, a designated trainer within the worker s department, or the worker s supervisor 6.1.2 Training whenever hazardous chemicals in the workplace change or use of the hazardous chemical changes, typically provided by the worker s supervisor Training will include: 6.1.3 Information on the hazardous chemicals in the worker s area 6.1.4 Process and operations where hazardous chemicals may be encountered 6.1.5 Location and availability of Indiana University s written Hazard Communication Program 6.1.6 Location and availability of the list of hazardous chemicals for that work area 6.1.7 Location and availability of the MSDSs for the hazardous chemicals for that work area 6.1.8 How to read and understand the MSDSs and chemical labels 6.1.9 Physical and health hazards of the hazardous chemicals in the work area 6.1.10 Methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area 6.1.11 Measures employees can take to protect themselves from these hazards e.g., appropriate work practices, personal protective equipment to be used, and emergency procedures 6.1.12 Details of the 6.1.13 An explanation of labeling systems and MSDS content 6.1.14 How to obtain and use the appropriate hazard information 6.1.15 Any special procedures or processes, such as engineering controls, work practices, and use of personal protective equipment that are intended to protect the worker from the hazards 6.2 Recordkeeping: A record will be kept of each employee s training on the Hazard Communication Program. Training records will include: 6.2.1 Employee s name 6.2.2 Employee s signature 6.2.3 Training topic and brief summary of content 6.2.4 Date and location of training 6.2.5 Training instructor s name Page 4

APPENDIX A: DEFINITIONS Indiana University Office of Environmental, Health, and Safety Management Acute: Health effects which show up a short length of time after exposure and runs a comparatively short course. Administrative Controls: Methods of controlling employee exposures by use of appropriate procedures, job rotation, or limited work periods. Air Monitoring: The sampling for and measuring of pollutants or hazardous chemicals in the atmosphere. ACGIH: American Conference of Governmental Industrial Hygienists Carcinogen: A substance which is known to cause cancer. Ceiling Limit: An airborne concentration of a toxic substance in the work environment, which should never be exceeded. CFR: Code of Federal Regulations Chemical: Any element, chemical compound, or mixture of elements and/or compounds Chemical Manufacturer: A business where chemicals are produced for use or distribution of chemicals on a commercial basis. Chemical Name: The scientific designation of a chemical in accordance with the nomenclature of the International Union of Pure and Applied Chemistry (IUPAC) or the Chemical Abstracts Service (CAS) or a name which clearly identifies the chemical for the purpose of conducting a hazard evaluation. Chronic: Health effects which show up a long time after exposure and are persistent or prolonged. Combustible: A material having a flashpoint at or above 100 o F. Common Name: Any designation such as a trade name, brand name or generic name used to identify a chemical other than by its chemical name. Compressed Gas: A chemical substance which at standard temperature and pressure is a gas, but is contained under pressure. Confined Space: An enclosure that is difficult to get out of, has limited or no ventilation, and is not intended for human occupancy. Container: Any bag, barrel, bottle, box, can, cylinder, drum, reaction vessel, storage tank, or the like that contains a hazardous chemical. Page 5

Corrosive: A substance which can cause physical change, usually deterioration or destruction, particularly to human tissues. Cryogenic: A substance held at very low temperatures, typically below -150 o C. Distributor: A business which supplies hazardous chemicals to other distributors or businesses. Dose: The amount of a substance delivered in a specified time (amount x time). Employee: A worker who receives monetary compensation from Indiana University and who may be exposed to hazardous chemicals under normal conditions or in foreseeable emergencies. Employer: Indiana University and its representatives. Explosive: A material that causes a sudden, almost instantaneous release of pressure, gas, and heat when subjected to sudden shock, or pressure, at high temperature. Exposure: Contact with a chemical, biological or physical hazard. Flammable: A substance with a flashpoint below 100 o F. Flashpoint: The minimum temperature at which a liquid gives of a vapor in sufficient concentration to ignite. Foreseeable Emergency: A potential occurrence such as, but not limited to, equipment failure, rupture of containers, or failure of control equipment that could result in an uncontrolled release of a hazardous chemical into the workplace. Hazardous Chemical: Any chemical that has the capability to produce adverse effects on the health or safety of humans. Hazard Warning: Words, pictures, symbols or combination thereof that conveys the specific physical or health hazard of the chemical. Health Hazard: A chemical for which there is statistically significant evidence based on established scientific principles that acute or chronic health effects may occur in exposed employees. IDLH: Immediately Dangerous to Life or Health: a very hazardous atmosphere that can cause serious injury or death to an exposed employee. Immediate Use: The material will be used only within the work shift and will be under the control of and used only by the designated worker. Page 6

Irritant: A substance that produces an irritating effect when it contacts skin, eyes, or respiratory system. Label: Written, printed, or graphic material displayed on, or affixed to containers of hazardous chemicals. LD 50 : The dose required to produce death in 50% of the subjects in a specified time. Lower Explosive Limit: The lower limit of flammability of a gas or vapor at ambient conditions expressed in percent of the gas or vapor. Material Safety Data Sheet (MSDS): Written or printed material concerning a hazardous chemical which is prepared in accordance with IOSHA s Hazard Communication Standard, 29 CFR 1910.1200 Mixture: Any combination of two or more chemicals if the combination is not in whole or in part the result of a chemical reaction. NIOSH: National Institute for Occupational Safety and Health: the federal agency charged with research in the areas of occupational safety and health. Oxidizer: A chemical other than a blasting agent or explosive as defined in 29CFR1910.109(a) that initiates or promotes combustion in other materials, thereby causing fire either of itself or through the release of oxygen or other gases. Permissible Exposure Limit (PEL): The quantity of a hazardous material to which a worker can be exposed for up to eight hours per day or 40 hours per week without experiencing any ill effects. Personal Protective Equipment (PPE): Devices worn by the worker to protect against hazards in the work environment including respirators, gloves, hearing protection, and/or eye protection. Physical Hazard: A chemical for which there is scientifically valid evidence that it is a combustible liquid, a compressed gas, explosive, flammable, an organic peroxide, an oxidizer, Pyrophoric, unstable (reactive) or water-reactive. Poison: Any substance which when taken into the body is injurious to health. Reactivity: A chemical s susceptibility to undergoing a chemical reaction or change that may result in dangerous side effects, such as explosion, burning, and corrosive or toxic emissions. Route of Entry: The path by which chemicals can enter the body: inhalation, ingestion, and absorption. Page 7

Sensitizer: A material that can cause an allergic reaction of the skin or respiratory system. Repeated exposures to sensitizers evoke an increasingly more serious allergic response. Short-term Exposure Limit (STEL): The maximum concentration of a substance to which a worker can be exposed to for a short period of time, typically 15 minutes. Target Organ Effects: The organ-specific effects of a given hazardous chemical. TLV: Threshold Limit Value: Identical to PEL. Toxic: A property of a substance making it harmful to living organisms. Upper Explosive Limit: The highest concentration (expressed in percent vapor or gas in the air by volume) of a substance that will burn or explode when an ignition source is present. Page 8

Instructions for Requesting Prescription Safety Eyewear and Completing Request Form All departments and employees covered by the Indiana University Safety Eyewear Program must use this form to request prescription safety eyewear. The Indiana University School of Optometry will not issue eyewear without a completed form. The purpose of the form is to: Verify that a Job Hazard Assessment (JHA) has been completed and safety eyewear is required for the job. Provide departmental account information for billing purposes. Assist in tracking safety eyewear issued to employees. To complete the form: Supervisor to provide employee and billing information and appropriate department signatures. Form must be legible. Please print clearly. Provide employee s name as officially listed with the university. Select and circle Section IA or IB as appropriate. For IA, new glasses, select a, b, or c. If b or c is selected, complete describe options section. Indicate whether a JHA has been completed or not. Include billing information and approval signatures. Forward completed form to IU EHS, 1514 E. Third Street, and Attention: Safety Eyewear Program Coordinator. Safety eyewear program coordinator will verify that a JHA has been performed and is current. If not, he/she will schedule and perform a JHA before approving request. Make copy of form for EHS records. Return original form to employee s manager. Employee s manager will forward form to employee (maintaining any desired records). Employee will schedule appointment for eye exam, if needed, and for obtaining eyewear with the IU School of Optometry. Employee will meet with IU School of Optometry personnel to select eyewear and have same properly fitted. Exceptions: If the EHS job hazard analysis indicates that safety eyewear is NOT required, the department may choose to purchase the safety eyewear for the employee anyway. If so, EHS will sign the form and indicate (in comments section) that the eyewear is not a safety requirement. Any employee may purchase safety eyewear, whether required by job hazards or not. In this case, the employee will be responsible for all costs. Page 11
