INVITATION FOR QUOTATIONS FOR CALIBRATION AGENCY UNDER NATIONAL SHOPPING PROCEDURES To, Project Director, Uttar Pradesh Health System Strengthening Project, Training Block-2, State stitute of Health & Family Welfare C- Block, dira Nagar, Lucknow-226016 (U.P.) Dear Sir, Sub: INVITATION FOR QUOTATIONS FOR CALIBRATION AGENCY UNDER NATIONAL SHOPPING PROCEDURES 1. You are invited to submit your most competitive single quotation for calibration of the equipment s mapped with locations and installed at District Level hospitals as mentioned in Annexure 1 2. Government of dia has received a credit from the ternational Development Association (IDA) in various currencies equivalent to US$ 152 million towards the cost of the Uttar Pradesh Health System Strengthening Project and intends to apply part of the proceeds of this credit to eligible payments under the contract for which this invitation for quotations is issued. 3. Bid Price a) The contract shall be for the full quantity as described above. b) Service Tax should not be included in the total price. c) The rates quoted by the bidder shall be fixed for the duration of the contract and shall not be subject to adjustment on any account. d) The Prices shall be quoted in dian. 4. Each bidder / organization shall submit only one quotation with one bid price in INR. 5. Validity of Quotation Quotation shall remain valid for a period not less than 120 days after the deadline date specified for submission. 6. Evaluation of Quotations The Purchaser will evaluate and compare the quotations determined to be substantially responsive i.e. which a) are properly signed b) conform to the terms and conditions, and specifications as mentioned in Annexure 2 Page 1 of 11

c) bidder must submit & produce documents defining PAN number & TIN number & Service Tax number d) The Quotations would be evaluated for complete set. Quotations offering part quantity will be rejected. 7. Award of contract The Purchaser will award the contract to the bidder whose quotation has been determined to be substantially responsive and who has offered the lowest evaluated quotation price. a) The contract will be valid for 2 years with renewal in the 2nd year basis the performance in the first year b) The Purchaser will award the contract to the bidder whose quotation has been determined to be substantially responsive and who has offered the lowest evaluated quotation price. c) The payment will be made to the agency basis the unit rate quoted for respective equipment d) If the calibration is required on the same equipment within 6 months then the agency will undertake the same without any extra cost e) Total contract will be awarded to the agency which quotes the minimum for the entire set of equipment's. 7.1 Notwithstanding the above, the Purchaser reserves the right to accept or reject any quotations and to cancel the bidding process and reject all quotations at any time prior to the award of contract. 7.2 a) The bidder whose bid is accepted will be notified of the award of contract by the Purchaser prior to expiration of the quotation validity period. The terms of the accepted offer shall be incorporated in the supply order. b) The Performance Security acceptable to the Employer shall be the in the Standard Form of Bank Guarantee A Bank Guarantee for an amount of 8% of the contract price (in favour of Project Director UPHSSP) it should be valid till 15 months from the date of signing of contract. case the contract is renewed then fresh/renewal of the Bank Guarantee or FDR is required for a period of next one year. c) The Service Provider shall provide the Performance Security to the Employer no later than the date specified in the Letter of acceptance. The Performance Security shall be issued in an amount and in form of Bank Guarantee or Fixed Deposit Receipt by a bank nationalized / scheduled bank and denominated in the currency in which the Contract Price is payable. 8. Payment shall be made after minimum 30 working days of receipt of voice at UPHSSP. 9. You are requested to provide your offer latest by 1300 hours by 29th June 2015 10. No. : UPHSSP/QA/CALIBRATION AGENCY/2015-16/01/CN1 Uttar Pradesh Health Systems Strengthening Project TERMS OF REFERENCE FOR CALIBRATION AGENCY OBJECTIVE: To perform calibration services, safety testing of biomedical equipment in accordance with current standards, codes and procedures. PURPOSE: Testing and calibration of the Bio medical equipment is proposed to be done to ensure quality control of equipment. Calibration is the process of evaluating and adjusting the precision and accuracy of measurement equipment. Calibration of equipment would ensure accuracy in diagnosis and effectiveness in treatment and developing care plan of the patient in hospital setting. Proper calibration of an instrument allows people to have a safe working environment and produce valid data for future reference. The instrument reads in the same units as the reference standards. The purpose of the calibration is to identify and eliminate any bias in the instrument relative to the defined unit of measurement. To ensure medical devices / equipment are safe and reliable and that they operate at the required level of performance in support of quality patient care. When do instruments need to be calibrated? Before major critical measurements Before any measurements that requires highly accurate data, send the instruments out for calibration and remain unused before the test. After major critical measurements. Send the instrument for calibration after the test helps user decide whether the data obtained were reliable or not. Also, when using an instrument for a long time, the instrument's conditions will change. After an event. The event here refers to any event that happens to the instrument. For example, when something hits the instrument or any kinds of accidents that might impact the instrument's accuracy. A safety check is also recommended. When observations appear questionable. When you suspect the data's accuracy that is due to instrumental errors, send the instrument to calibrate. Per requirements. Some experiments require calibration certificates. Testing and calibration of the equipment is to be done from recognized calibrating agencies those are expertise in dealing with Bio medical equipment s. strument calibration is intended to eliminate or reduce bias in an instrument's readings over a range for all continuous values. Precision is the degree to which repeated measurements under unchanged conditions show the same result. Accuracy is the degree of closeness of measurements of a quantity to its actual true value. For this purpose, reference standards with known values for selected points covering the range of interest are measured with the instrument in question. Then a functional relationship is established between the values of the standards and the

corresponding measurements. The instrument reads in different units than the reference standards. The purpose of the calibration is to convert the instrument readings to the units of interest. Job Description: The agency will: Perform calibration services on specified biomedical equipment/medical devices in accordance with the known standards. Adjust internal electronics for direct read so no correction is required Verify performance with electronic calibration Make mechanical adjustment as required Maintain and update the equipment inventory (including stored faulty equipment) by adding information on the status of equipment, maintenance history, where known, routine maintenance requirements and expected timing of replacement of all biomedical equipment and medical devices. Prepare a recommended list of spares for essential medical equipment. spect and test biomedical equipment medical devices to ensure compliance with manufacturer s specification and standards. Perform other related duties as assigned and as requested by the Ministry of Health and Social Services. Calibration is done at least once in a year hence calibration sticker after successful completion of the calibration to be pasted on the equipment surface which should include Name of the Equipment, Calibration date, Due date of calibration, department, Name of the agency. Qualification & Other Requirements 1. Must be an accredited calibration agency under the Nation Accreditation board for testing and calibration Laboratories (NABL) 2. Calibration of the Bio medical equipment is to be carried out by the Bio medical Engineer. 3. Agency interested in taking up the task should provide key parameter of testing and calibrating bio medical equipment, Traceability Certificates of its Master Calibrator and conclude by documenting the test results and issuing a calibration certificate for Bio medical equipments. Skills & Experience required for the Agency: Must be experienced in the repair, calibration and use of general bio-medical equipment Experience in the repair, calibration and use of x-ray and imaging equipment. Experience in the Operating Theatre environment and equipment associated with this area. Reporting Requirements of the Agency: Agency will report to the CMS /SIC of the Hospital. An updated equipment calibration history should be submitted to the CMS/SIC of Hospital reporting progress against the work plan and recommendations for further service development and improvements including any additional training required for hospital staff.
