Overview The QM Report: Provides resident specific details for the CMS Quality Measures (QM) Displays Short Stay and Long Stay Measures with drill down capability Surveyor Measures are included Enables a proactive approach to quality improvement incorporating QM data 4
QM Report Navigation Accessing Online Reports At the facility level, the QM Reports provide the following: Facility Summary Resident Summary Drill down Monthly Trend Report Resident Roster 5
Access the QM Reports 1. Click the Location tab to go to the Facility Lookup page 2. Click the name of your Facility to view the Facility Menu 3. Select Products, and click to view the Products page 6
Access the QM Reports (cont.) 4. Select the name of the report you want to see from the Products list. We ll start with the QM Summary report. 5. Click to view the QM Summary 7
Access the QM Reports (cont.) The QM Reports are also accessible from the Products menu. 1. Roll the mouse icon over the Products Tab. The Products dropdown menu appears. 8
QM Summary Report Click to select the time period Click to view list of triggering residents The QM Summary Report displays a summary of QM triggers for the facility, based on a rolling quarter. The four Surveyor Measures are indicated with an asterisk (*). 9
QM Summary Report Details Numerator: the number of residents in the facility who triggered for the measure Denominator: the total number of residents who qualify for the calculation Percent (%): the percentage of residents in the facility who triggered the QM Adjusted Percent (Adj %): the result of additional resident risk adjustment Adjusted measures: New/Worsened Pressure Ulcers (SS); Pain (LS); Catheter (LS) National Average: the PointRight national average Percentile: the facility s rank in comparison to the PointRight National Average benchmark 10
QM Summary Report (cont.) Clicking on the name of a measure will open a new browser window displaying the list of residents who triggered during the report period 11
QM Summary Report (cont.) Clicking a resident s name will display the resident s MDS History for that measure in a new browser window Highlighted rows indicate triggering assessments Roll the mouse icon over an MDS item to see details 12
QM Summary Report (cont.) Flags appear for Percentile rank of 75 or above A lower percentile rank is considered better performance 13
View the MDS Assessment Click the ARD to view a PDF copy of the MDS 14
Export the QM Report to Excel Click the Export To Excel button to create a spreadsheet of the QM Summary report 15
Print the QM Reports Roll the mouse icon over the Print tab Select Print Report from this Page A printable version of the report page will be generated in a new window. Click the Print button in the printable window to print. 16
QM Trend Report Select time period and measure The QM Trend Report shows your facility s 24 month trend in each measure. Your facility s performance is shown in BLUE The PointRight National Benchmark is shown in RED Click a point on the graph to view the list of residents who triggered 17
QM Resident Roster Click to export to spreadsheet Click a column header to sort by that measure Roster is divided into multiple pages on the screen display 18
