Case Study - Fall from a Height IP Information Age: 64 Gender: Male Job Title: IST Accident Information On 06/12/2016 IP was working on a step ladder behind the deli counter at around 2:40pm, inspecting a faulty lighting fitting 600 x 600mm in size that appeared to look loose in a suspended ceiling grid. After inspection the fitting was found to be faulty and requiring work to make safe, which involved the light fitting being manoeuvred out of the grid and into the ceiling void. Whilst working on the step ladder the IST lost his balance, fell backwards (off the step ladder) hitting his head, hand and back on the tiled floor. Additional Information An ambulance was called immediately (arrived at 15:00) but not needed due to the nature of the injuries shown, however due to the persistence of pain, the IP was driven to the hospital by the GMM.

2 Witness Statements Witness Statement #1 The IP was working up the ladder behind the deli counter when I started my shift. He was working on the lights. He was very unsteady, his hands were shaking and also his legs. I mentioned he was unstable to Witness #2. He was stood at the very top of the ladder. I heard a noise whilst I was working and looked to see the IP fall backwards onto the floor. He landed on his back and his head hit the floor. The IP was working on his own Witness Statement #2 At 2:30pm, when I finished my break, I saw the IP doing maintenance work on the light above the deli counter. I could see he was struggling so I held the ladder as he climbed down with the light. The light was then pulling at the wire as he rested it against the ladder so I moved away and watched him put bolts in the side of the light. After it was fixed he then climbed back up to insert the light back into place. By this time I was using the slicer and I could see he was unsteady on the ladder. I stopped and went to help by holding the ladder; he stepped up to the top plate and tried to position the light into its tiling, but it was too heavy for him. I went to support him and hold the light with my right arm as he regained balance but his natural shaking, together with the weight of the light, must have knocked him off balance too much as his feet gave way. He fell backwards and as far as I know it was a bit of a blur. I don t remember exactly how he fell but he hit his head flat against the floor tile. I saw blood splatter so I rushed to the customer service desk to grab a blanket to rest and compress his head wound. He was still fully conscious. The IP was responding fine; had no vision or memory difficulty and the only pain he reported (aside from his head cut) was a pain in his ribs behind his shoulder. This makes me believe he hit his side against the steel sides, however I m not sure as I didn t see exactly how the IP fell or landed.

The light fitting was lifted out of the ceiling grid and manoeuvred to put the 2 support arms. It was at this point the IP lost his balance and fell backwards
