4 Getting Started Course Goals By completing this course, you will be able to: Filter the Action List to display Notifications and Accretions. View the queue for all Notifications and Accretions and resolve all discrepancies in CROWNWeb. Assign an Action to Notifications and Accretions after the discrepancy has been resolved.

5 Getting Started Before You Begin Before you can complete this procedure, the following conditions must be met: You must be a Facility Editor to: View notifications and accretions that are within your scope. Mark notifications and accretions as Under Investigation. Accept or Reject notifications and accretions. Escalate notifications and accretions to the Network.

6 What are Notifications and Accretions? Notification A discrepancy in patient data between what the REMIS database has received from another CMS database and what exists in CROWNWeb (e.g., date of birth, last name, or a transplant that wasn t recorded in CROWNWeb). Accretion A patient the REMIS database has learned about from another database and believes to be ESRD, but has not been admitted into CROWNWeb as a patient.

7 What are Notifications and Accretions? (CONT.) Notifications and Accretions provide the ability for facilities to use CROWNWeb to troubleshoot and correct discrepancies generated by REMIS. Facility B NOTIFICATION ACCRETION NOTIFICATION NOTIFICATION ACCRETION ACCRETION REMIS is a database that gathers information from various CMS databases, and compares that data to CROWNWeb, the system of record for ESRD. CROWNWeb receives discrepancy information each night, and assigns Notifications and Accretions based on that information.

8 Resolving Notifications and Accretions All facilities (including batch submitting organizations) are responsible for correcting and maintaining their own data. All corrections to the data must be made through CROWNWeb. New notifications and accretions should be resolved within 15 days and notifications and accretions that are under investigation are to be resolved within 30 days. Networks are responsible for the oversight of notifications and accretions and will help the facilities. Prisons, Veterans Administrations, and transplant facilities are not currently required to use CROWNWeb.

9 Notification Action Types There are two Notification Action Types you can work or resolve in CROWNWeb: REMIS Identity Notification: Identifies differences with a patient's identity (i.e., first name, last name, date of birth, gender, Medicare Claim Number*, and SSN). When you update this type of notification, CROWNWeb is automatically updated. REMIS Event Notification: Identifies any treatment differences in modality, or the enrollment data base has received notice that the patient has passed away. A warning message displays that you have to update the Admit/Discharge and Treatment records before accepting the notification. *NOTE: Click here for an explanation of Medicare suffixes.

17 Accretion Action Types There is only one Accretion Action Type you can work or resolve in CROWNWeb: REMIS Accretion: Identifies a patient that exists in the REMIS database, but is not admitted to a facility in CROWNWeb. To accept this accretion, you must admit the patient to your facility.

25 Troubleshooting Commonly experienced problems, recognized solutions, and other key information: For an accretion admit, some (but not all) of the information will auto-populate. Always review, verify, and enter any information, as required. Changes are automatically made for patient identifiers, but treatment updates/additions must be made in CROWNWeb by the user. The information displayed on the View screen is what was in CROWNWeb at the time the notification was loaded, not what is currently in CROWNWeb. Comments, if added, will be maintained in the Comment History field.

26 Summary To work and resolve Notifications: 1. From the Home Page, click Patients. 2. From the Search Patients screen, click the Action List link. 3. Filter by Action Type for Notifications. 4. Select a facility from the Facility DBA Name field. 5. Click Submit. 6. Select a Notification link. 7. For Treatment Notifications, manually add the treatment information to the patient s record. 8. Select an option in the Action field, or click Escalate. 9. Add any comments in the Comments field. 10. Click Submit. 11. Click the Ignore Warning checkbox and click Submit again. 12. Click the patient s CROWN UPI to review updates.

27 Summary (CONT.) To work and resolve Accretions: 1. From the Home Page, click Patients. 2. From the Search Patients screen, click Action List. 3. Filter by Action Type. 4. Select a facility from the Facility DBA Name field. 5. Click Submit. 6. Select an Accretion link. 7. Add Comments (optional). 8. Click an Action button: Accept, Reject, Investigate, or Escalate. 9. If Accept is clicked, admit the patient in CROWNWeb.
