1 INSTRUCTIONS Please answer the questions that are applicable with as much detail that you can remember. The details are important! If a question does not apply to your experience simply write N/A for the answer to that particular question. Southwest Ghost Hunter's Association Page 1

5 What was the weather like that day? Was there any visible lightning? Was thunder heard? Was there any form of precipitation (rain, snow, hail, fog, mist, etc.)? Was there any kind of electrical problems before, during or after the occurrence? Was there any sort of temperature variation associated with the occurrence? What you believe happened? Was the witness sleeping before the event? Was he or she tired prior to the events? Did the witness call for help or scream during the event? Did you suddenly feel nauseous or fatigued? 6 Did you have any form of caffeine before the event (coffee, energy drinks)? If so how much? Were there any animals present at the time of the event? If Yes: List them. How did the animal(s) react? What was the behavior of the animal(s) like throughout the day How did the animal(s) act following the event? Did any objects break before, during or after the event? If Yes: Describe. 7 Did anything else happen that was out of the ordinary? Visual/Apparitions Describe the apparition with as much detail as possible. How far away from the apparition was the witness?
