SUBJECT: Monitoring Well Installation Report, Entergy White Bluff, Class 3N Landfill, Permit No S3N-R3, AFIN: FTN No. R On behalf of Entergy Services, Inc. (Entergy), FTN Associates, Ltd. (FTN) is providing this technical memorandum for the Entergy White Bluff Class 3N Landfill in Redfield, Arkansas. The memorandum presents a summary of field activities associated with the installation and decommissioning of monitoring wells and piezometers as required by Arkansas Pollution Control and Ecology Commission (APCEC) Regulation No. 22, 22.13(f). 1.0 BACKGROUND Entergy submitted two work plans to the Arkansas Department of Environmental Quality (ADEQ) for the proposed installation of seven monitoring wells and decommissioning of two piezometers located at the landfill, in accordance with APCEC 22.13(f). The proposed scope of work in each work plan and associated ADEQ approvals are summarized below. To meet APCEC Regulation No. 22 requirements for the future groundwater monitoring program at the landfill, Entergy proposed the installation of four additional monitoring wells and the decommissioning of two temporary piezometers in a work plan dated January, 17 (ADEQ document identification number [DIN] 937). The work plan described the proposed installation of two shallow wells, MW-8S and MW-9S, screened in the saturated sediments of Stratum I, and two deep wells, MW-12D and MW-13D, screened in the saturated Stratum III soils. The proposed locations of these wells and the decommissioning of temporary piezometers FTN PZ-4 and FTN PZ-6 were based on prior communication between ADEQ, Entergy, and FTN.

3 Tammie J. Hynum May 31, 17 Page 2 Entergy submitted a second work plan dated January, 17, for the proposed installation of three additional Stratum III wells: MW-D, MW-14D, and MW-15D (DIN 938). These wells were being installed to augment the landfill's monitoring program under Title Code of Federal Regulations (CFR) Part 257, 257. through A conditional approval for the two work plans was granted on February 2, 17, in a letter from ADEQ (DIN 967). In the letter, ADEQ requested a shallow well be installed near proposed well MW-12D in lieu of the proposed shallow well MW-9S if Stratum I sediments were found to be saturated during the installation of proposed well MW-12D. ADEQ also requested that the location and possibly the depth of MW-13D be modified. All other proposed work described in the work plans (DINs 937 and 938) was approved. Entergy submitted a revised work plan on February 9, 17 (DIN 5), to address ADEQ s comments in the conditional approval letter (DIN 967). The revised work plan included the installation of a shallow well (MW-12S) near proposed Stratum III well MW-12D (instead of MW-9S near MW-9D) if Stratum I sediments were found to be saturated during installation of MW-12D. The revised work plan also relocated proposed MW-13D nearer to temporary piezometer FTN PZ-4. ADEQ approved the revised work plan (DIN 5) in a letter to Entergy dated February 16, 17 (DIN 718). 2.0 DRILLING AND WELL INSTALLATION Field activities associated with well installations were conducted February 8 through 25, 17. Walker-Hill Environmental, Inc. (WHE), an Arkansas-licensed driller, performed the drilling and well construction tasks, with an FTN geologist on-site to document the work. Soils for each well boring were described and classified according to the Unified Soil Classification System by the on-site geologist. Each Stratum I and Stratum III well boring was advanced using sonic drilling techniques and a 4x6-inch core and case system. Soils were continuously sampled using a -foot, 4-inch-diameter core barrel fitted with a sand catcher to maximize sample recovery. A -foot, 6-inch-diameter override casing was advanced over the core barrel prior to core retrieval. Where applicable, a temporary -inch-diameter casing was advanced 1 to 5 feet into the Stratum II clays during the drilling of Stratum III wells. This outer casing was left in place during deep well borehole advancement and during well construction activities to prevent or minimize downward migration of Stratum I groundwater. The large outer casing was not used during the drilling or construction of MW-13D because Stratum I soils are absent at that location.

Hynum May 31, 17 Page 3 Well construction procedures adhered to methods described in the Handbook of Suggested Practices for the Design and Installation of Ground-Water Monitoring Wells (US Environmental Protection Agency [EPA] 1991) and those required by the Arkansas Water Well Construction Commission (AWWCC). Each Stratum I well was constructed using 2-inch-diameter, flush-threaded, solid Schedule PVC riser and a -foot, 0.0-inch slotted, flush-threaded well screen and end cap. A filter pack consisting of - to -silica sand was placed in the well annulus to a minimum depth of 3 feet above the top of well screen, followed by a minimum of 3 feet of bentonite pellet seal and well-mixed bentonite-cement grout. The new Stratum I wells were secured at the surface with a 4-inch-square locking protective aluminum casings, concrete pads, and four concrete-filled pipe bollards. The Stratum III wells were similarly constructed except that 4-inch-diameter riser, well screen, and end cap were used and protected by an 8-inch-square locking protective aluminum/steel casing. The well identifications were clearly identified on the outside casings of each new well. Boring log and well construction details are included in Attachment PIZOMENTER ABANDONMENT Piezometer abandonment for FTN PZ-4 and FTN PZ-6 were performed by WHE with an FTN geologist on-site to document the work. Abandonment activities adhered to procedures recommended by EPA (1991) and those required by AWWCC. The temporary fence posts and protective netting around each piezometer were removed prior to abandonment activities. The 1-inch-diameter PVC casing, well screen, and end caps were removed by overdrilling using sonic drilling techniques and a 4-inch-diameter core barrel. The boreholes were backfilled using slow-release bentonite pellet plugs from the total drilled depth to the ground surface. Specifically, FTN PZ-4 was backfilled from a total depth of 45 ft below ground surface (bgs), and FTN PZ-6 was backfilled from ft bgs. The pellets were added slowly and periodically tamped to prevent bridging. Potable water was added following pellet installation to hydrate the pellets in place. 4.0 SURVEY Each new monitoring well was surveyed by Harmon Surveying, Inc., an Arkansas-registered professional surveyor. A site map depicting the surveyed locations of the new wells and the certified April, 17, survey report are included as Attachment 2. The survey report includes the horizontal and vertical coordinates based on Entergy s site-referenced coordinate system. The report also provides horizontal coordinates in the Arkansas State Plane Coordinate System, North American Datum of 1983 (NAD 83). The site-referenced vertical coordinates are equivalent to the North American Vertical Datum of 1988 (NAVD 88). The surveyed elevation data and general well construction details for each new well are provided in Table 1.

Immediately following this procedure, the wells were purged to remove the added water. This method was used because conventional methods of well development, such as surging and overpumping, are ineffective due to the low-yielding capacity and high-fines content of the target zone sediments. FTN developed the wells February 15, 17, through March 2, 17. Well development records are included as Attachment 3.
