Negotiations with the consultant have resulted in the attached Scope of Work, Exhibit G and Fee Summary, Exhibit H. This project is designated to be under the Equal Business Opportunity Program with a 15% JSEB Participation Goal. JSEB firms to be utilized to meet the Participation Percentage Plan for this contract are detailed on a separate attachment. Accordingly, this is to recommend that Contract No. 9588, originally executed April 6, 2011, between the City and Adkinson Engineering, P.A./Five Points Design Group, Inc. SCOPE OF PROJECT A The intent of the project is to furnish post design services for the proposed drainage improvements for the Noroad I Lambing Road area. These improvements have been previously designed and will be constructed. The purpose of this addendum is to increase the scope to include such miscellaneous post design services including pre-construction meeting, shop drawing review, periodic site visits during construction, responding to design clarification requests, and final certifications. II. NOTICE TO PROCEED No work on this portion of the project shall be performed until this addendum has been executed and a Notice to Proceed is issued which specifically authorizes such work. 1
